药物性牙龈增生牙周非手术治疗的疗效观察

Effect of non-surgical periodontal therapy on the drug-induced gingival proliferation

目的评价单纯牙周非手术治疗对钙拮抗剂类药物导致的牙龈增生的临床疗效。方法选取钙拮抗剂类药物导致的牙龈增生患者18例,男11例,女7例,在未停药的情况下进行牙周非手术治疗,在牙周非手术治疗前和治疗后1个月、3个月、6个月记录牙龈增生指数、菌斑指数、出血指数和探诊深度并与初诊时对照观察疗效。结果牙周非手术治疗1个月后与基线相比,探诊深度、出血指数和牙龈增生指数出现了较明显的改善,经统计学检验有显著差异(P<0.01),菌斑指数无显著差异(P>0.05)。牙周非手术治疗3个月、6个月后与基线相比牙龈增生指数、菌斑指数、出血指数和探诊深度均有显著性差异(P<0.05或P<0.01),治疗后时间越长,各临床指数减轻越明显。结论对于正在服用钙拮抗剂类降压药的患者,牙周手术治疗可改善钙拮抗剂药物引起牙龈增生的程度。

Objective To evaluate the clinical efficacy of simple non - surgical periodontal therapy on gingival proliferation induced by calcium channel blockers. Methods Eighteen patients （ 11 males and 7 females） with gingival proliferation induced by the calcium channel blockers were treated with non - surgically periodontal therapy. Hyperplasia indexes （HI）, plaque indexes （PLI）, bleeding indexes （BI） and probing depth （PD） were recorded at baseline, 1, 3 and 6 months respectively. Change from baseline in HI, PLI, BI and PD was assessed at 1,3 and 6 months, respectively. Results The data showed that the efficacy of non - surgically periodontal therapy increased with time. Non - surgically periodontal therapy significantly improved HI, PI, PLI and PD after 3 - month treatment when compared with that of baseline （ P 〈 0.05 ）. Conclusion For those who are taking calcium channel blockers, non - surgically periodontal therapy may inhibit the gingival proliferation.
