摘要
目的:探讨P-糖蛋白(P-glycoprotein,P-gp)在葡萄胎(Hydatidiform mole,HM)组织中的表达,分析其表达与滋养细胞增生和血清β-HCG定量的关系。方法:应用免疫组织化学方法,对50例完全性葡萄胎病人第1次清宫所得组织进行P-gp染色。结果:滋养上皮增生程度与P-gp表达,随着细胞增生强度的增高,细胞的染色强度减弱(P<0.01);血清β-HCG定量与P-gp表达也呈负相关(P<0.01);6例发展成侵蚀性葡萄胎的病例中,4例血清β-HCG定量达170000mIU/ml以上,滋养细胞中P-gp的表达均为弱阳性。结论:葡萄胎病人的绒毛组织解毒屏障功能,可能较正常绒毛减弱;滋养细胞中P-gp表达明显减弱者可能和日后发展成侵蚀性葡萄胎有一定关系。
Objective: To investigate the expression of p - glycoprotein ( p - gp) in trophoblastic cells of hydatidiform mole and to analyze the relationship between p - gp expression and proliferation of trophoblastic cells as well as sera β- HCG. Methods: Immunohisto- chemical method was used to detect the expression of p - gp in 50 cases of hydatidiform mole. All the tissue analysis was obtained from the first suction curettage. Results: As the degree of proliferation of trophoblastic cells increased, decreased expression of p - gp was observed (P 〈 0. 01 ) ; The amount of Sera b - HCG was inversely correlated with the degree of staining in trophoblastie cells (P 〈 0. 01 ) ; Among 6 cases who developed invasive mole, 4 cases presented a sera b - HCG superior to 170000mIU/ml, along with a weaker expression of p - gp. Conclusion: The weaker expression of p - gp in trophoblastie cells of hydatidiform mole with higher proliferation suggests that the detoxificafion function of trophoblastic cells possibly decreases than that of normal chorine. This may be correlated with the development of invasive mole.
