摘要 目的:探讨64层螺旋CT血管分析(VA)软件在脑动脉CTA的应用的可行性与局限性。材料与方法:32例自发性蛛网膜下腔出血患者完成64层螺旋CT数字减影CTA增强扫描后选取48段病变或可疑病变脑动脉分为两组。分别采用常规CYA图像(VR、MIP、MPR)和常规CTA图像+VA图像作诊断,诊断结果以DSA或手术结果为准,比较2组诊断的特异性和敏感性。结果:常规CTA结合VA对脑血管性病变诊断的特异性和敏感性均高于单独的常规CTA。VA对脑动脉瘤及脑动脉斑块的显示、软硬斑块的鉴别和血管腔狭窄程度的测量,效果优于常规CTA。结论:VA软件适用于脑血管性病变的诊断,具有很大的临床价值。多种重建方法综合应用是CTA正确诊断脑血管性病变关键。 Purpose: To investigate the diagnosis value of vessel analysis (VA) in intracranial 64 - row mulfisecfion CT angiography. Materials and Methods: Patients (n = 32) with spontaneous subarachnoid hemorrhage were underwent 64- row mulfisection CT angiography and 48 segments of abnormal vessel were studied. Diagnosis was done by two groups: the diagnosis group 1 was according to conventional CTA reconstruction (VR, MIP and MPR) and group 2 according to conventional CTA reconstruction and VA too. The diagnostic specificity and sensibility of two groups were compared according to the outcome of DSA or/and operation. Results: The diagnostic specificity and sensibility of group 2 were higher than group 1. Intracranial aneurismal necks measured, arteriosclerosis plaques differential diagnosis and angiostegnosis measured were all better done by VA than conventional CTA reconstruction. Conclusion: Sixty- four- row multisection CT DSCTA was more effective to elimination skull and made the diagnosis of aneurysms below Willis' circle easier, but normal CTA was also important. VA could also use in intracranial vascular lesions. Combine conventiona CTA and VA could made the diagnosis of intracranial vascular lesions more accurately.
