血清与胸水ADA、LDH、CEA联合检测在胸水鉴别诊断中的价值 被引量:4

摘要 目的探讨血清和胸水腺苷脱氢酶(ADA)乳酸脱氢酶(LDH)及癌胚抗原(CEA)联合检测在胸腔积液鉴别诊断中的价值。方法对88例结核性胸腔积液患者和36例癌性胸腔积液患者进行胸水和血清ADA和LDH及CEA的定量分析,胸水/血清ADA、胸水/血清LDH和胸水/血清CEA的比值,并进行统计学分析。结果两组胸水ADA、胸水/血清ADA比值、LDH、胸水/血清LDH胸水CEA、胸水/血清CEA比较均有显著性差异(P<0.01)。结论胸水ADA与胸水/血清ADA比值、胸水LDH与胸水/血清LDH、胸水CEA、胸水CEA/血清CEA比值联合检测具有互补作用,能提高诊断准确率。 Objective To discuss the value of combined detection of adenosine dehydrogenase (ADA), lactate dehydrogenase(LDH) and carcinoembryonic antigen(CEA) in serum and hydrothorax which used in identification and diagnosis with Pleural Effusion. Methods Quantitative analysis were carried out with serum and hydrothorax ADA, LDH and CEA of 88 cases of patients with tuberculosis pleural effusion, and 36 cases of patients with malignant pleural effusions, calculating the ratio of hydrothorax/serum ADA, hydrothorax/serum LDH and CEA of 88 cases of patients with tuberculosis pleural effusion, and 36 cases of patients with malignant pleural effusions, calculating the ratio of hydrothorax/ serum ADA, hydrothorax/serum LDHandhydrax/serum CEA, and implementing statistical analysis. Results The ADA in hydrothorax and the ratio of hydrothorax/serum ADA. the LDH in dhydrothorax and the ratio of hydrothorax/serum LDH, the CEA in hydrothorax and the ratio of hydrothorax/serum CEA of two group were significantly different (P〈 0.01). Conclusions The combined detection of ADA in hydrothorax and the ratio of hydrothorax/serum ADA,the LDH in hydrothorax and the ratio of hydrothorax/serum LDH, the CEA in hydrothorax and the ratio of hydroth orax/serum CEA have the complementary role and may improve the diagnostic accuracy.
