超声引导下经皮经肝穿刺胆管引流术在梗阻性黄疸中的应用

Application of Percutaneous Transhepatic Cholangial Drainage in Obstructive Jaundice under Supersonic Guidance

目的探讨超声引导下经皮经肝穿刺胆管引流术(PTCD)在梗阻性黄疸中的临床应用。方法自1997年4月至2007年6月在海门市三厂医院就诊的梗阻性黄疸30例在超声引导下行经皮穿刺胆管引流术。结果超声引导下经皮穿刺胆管引流1次成功率为80%(24/30),2次成功率为90%(27/30),3次成功率为96.7%(29/30),PTCD置管成功为26例,置管成功率为86.7%,仅1例出现并发症,术后出现出血,经充分引流并使用止血药物后消失。结论超声引导下经皮穿刺胆管引流准确率高,成功率高,是一种准确、安全、有效、简便的方法,对解除梗阻性黄疸有较高价值。

Objective To discuss the application of PTCD in obstructive Jaundice under supersonic guidance. Methods 30 cases with obstructive jaundice diagnosed in Sanchang hospital of Haimen from from April 1997 to June 2007 was included,they were all have PTCD. Results Once achievement rate of PTCD under supersonic guidance was 80% （ 24/30 ）, twice achievement rate of PTCD was 90% （ 27/30 ） , three times achievement rate was 96.7% （29/30）, only one case had complication of bleeding, which disappeared after enough drainage and medication of hemostyptic. Tubes were successfuly put into 26 cases, the achievement rate was 86.7%. Conclusion Under the supersonic guidance,the percutaneous transhepatic cholangial drainage had high accuracy rate and achievement rate ,which is the method with accuracy, safty, effectiveness and simple and has high value in reliefing obstructive jaundice.
