520例残留麻痹的急性弛缓性麻痹病例发病状况分析

ANALYSIS OF THE PREVALENCE OF 520 AFP PATIENTS WITH RESIDUAL PARALYSIS IN HEBEI PROVINCE FROM 2001-2006

[目的]了解我省消灭脊髓灰质炎(脊灰)后仍残留麻痹的AFP病例发病状况,为有效地控制麻痹病例的出现提供有价值的基础资料,巩固无脊灰成果。[方法]分析河北省2001～2006年520例残留麻痹的AFP病例病原学监测状况、流行病学特征,免疫史及疾病谱。[结果]2001～2006年AFP病例监测系统共报告AFP病例2516例,其中520例AFP病例在60d后仍残留麻痹,年平均发病率为20.7%,病例集中在小年龄组,﹤5岁的病例占60.0%,男性高于女性,夏秋季节流行高峰,6～8月份占病例总数的47.7%,520例AFP病例中不合格粪便标本采集率、未免疫或未全程免疫病例、脊灰病毒(PV)分离率均高于未残留麻痹的AFP病例,引起残留麻痹的疾病格-林巴利综合征(GBS)占48.3%,临床符合病例1例。[结论]加强残留麻痹的AFP病例监测,进一步提高合格粪便标本采集率和口服脊灰减毒活疫苗(OPV)的免疫覆盖率。

[Objective] To investigate prevalence of acute flaccid paralysis （AFP） patients with residual paralysis, so as to provide basic information for effectively controlling the incidence of paralysis and consolidating the achievement of poliomyelitis vaccine. [Methods] The monitored status of etiology, epidemiologie characteristics, immunization history and spectrum of diseases of 520 AFP patients with residual paralysis were collected and analyzed from 2001 to 2006 in Hebei. [Results] 2516 AFP patients were reported by the AFP surveillance system and 520 AFP patients also remain residual paralysis 60 days later with the annual incidence of 20.7%. The patients were concentrated in young people, and 60.0% of them were below the age of five veals. The incidence for male was higher than female, and the high risk seasons were summer and autumn. 47.7% of these patients attacked this disease during June to August. Out of 520 AFP patients. 48.3% of them showed to be disqualification samples of excrement, without immunity or whole range immunity, the isolation rate of PV to be higher than that of AFP patients without residual paralysis as well as with GBS of residual paralysis, but only one people was in line with the clinical presentation. [Conclusion] Efforts should be taken to strengthen the monitoring of AFP, and further improve the collection rateof qualified faeces and immunity rate of coverage of oral poliomyelitis live attenuated vaccine.
