移植肾功能延迟恢复的相关因素分析

目的研究影响本中心移植肾功能延迟恢复(DGF)发生的相关因素并寻找可能预测DGF发生的指标。方法对象为2004年7月至2005年12月在本中心接受同种异体肾移植手术治疗的150例受者,其中24例发生DGF;对可能影响DGF发生的各项指标进行统计学分析。结果受者术前透析类型、PRA水平、性别与DGF的发生无关;单因素和多因素线性回归分析发现DGF的发生与受者术后体内容量指标的变化存在线性关系;Cox风险比例回归模型提示术后第二个24h的尿量可以判断受者移植肾功能的恢复情况。结论本中心在有效控制DGF影响因素的情况下,发现移植受者术后体内容量的变化与DGF的发生密切相关;而且术后第二个24h尿量对于预测DGF是否发生有重要意义。

Objective To analyze the factors related to the incidence of delayed graft function （DGF） and explore the potential predictors to forecast DGF at our transplant center. Methods 150 recipients receiving cadaverie kidney transplantation between July 2004 and December 2005 were included and the records were statistically analyzed by the different regression models, Results Pretranplantation dialysis mode , PRA levels and recipient gender were not correlated with the incidence of DGF. The results of the tmivariate and multivariate regression analysis showed there was a positive relationship between the incidence of DGF and the variables related to post - transplant volume status. The results of the Cox proportional hazards regression analysis suggested urinary volume of the second 24 hour after transplantation was an independent predictor of DGF, Conclusions The incidence of DGF is influenced by post - transnlant volume status at our transplant center.Eurthermore,urinary volume of the second 24 hr is a valid predictor for DGF.
