摘要
目的评价酶联免疫斑点(Elispot)检测外周血中结核分枝杆菌性抗原特异性γ干扰素(IFN-γ)水平在肺外结核诊断中的价值。方法利用Elispot技术分别对24例肺外结核患者(肺外结核组)和97健康对照(对照组)外周血中产生结核菌抗原特异性IFN-γ的水平进行定量检测。对其中20例肺外结核病人和20例健康者同时采用澳大利亚CELLESTIS公司生产的全血干扰素试剂(QFT-G)进行IFN-γ分泌水平的平行检测。结果肺外结核组中IFN-γ水平和应答的阳性率显著高于对照组(P<0.05);Elispot、QFT-G检测肺外结核组和健康者的反应阳性率分别是:76.1%、84.2%,健康者中分别为10%、20%;两种方法检测结果没有统计学差异(χ2=1.1250,P>0.05)。结论Elispot试剂的检测结果与QFT-G基本一致,可用于肺外结核的辅助诊断。
Objective To investigate mycobacterium tuberculosis antigen specific IFN-γ response in patients with extra-pulmonary tuerbculosis(ETB)by Elispot and Quantiferon-TB-Gold (QFT-G) assay.Methods The levels of IFN-γ production in response to mycobacterium tuberculosis antigens in patients with extra-pulmonary tubereulosis(n=24) and healthy controls donors(n=97) were evaluated by an in-house Elispot. Comparative assay using Elispot and QTF-G was done in 20 patients with extra-pulmonary tuberculosis and 19 healthy controls.Results By using Elispot, the level of IFN-γ in ETB was much higher than that of the healthy controls ,and there was statistical difference between the ETB and the healthy controls (P〈0.05).The positive was observed in ETB and healthy controls were 76.1% ,84.2% and 10%,20% respectively.The comparative study indicated that there was no statistical difference between our inhouse Elispot and QTF-G assay (X^2=1.1250, P〉0.05).Conclusion Elispot assay is reliable for the diagnosis of ETB.
