肺癌组织中基质金属蛋白酶2及组织金属蛋白酶抑制剂2的表达与浸润、转移相关研究

摘要 目的探讨基质金属蛋白酶2(MMP-2)及组织金属蛋白酶抑制剂2(TIMP-2)在人肺癌中的表达及其与肺癌转移、预后的相关性,分析肺癌发生、发展、侵袭和转移的机制。方法应用链霉素抗生物素蛋白-过氧化物酶(S-P)法染色技术,检测60例肺癌及20例癌旁正常肺组织中MMP-2和TIMP-2的蛋白表达。结果肺癌与正常肺组织相比,MMP-2阳性表达率为73.3%vs 15.0%,TIMP-2阳性表达率为51.7%vs 10.0%(均P=0.000),肺癌的不同病理类型中MMP-2的阳性表达差异有统计学意义(P<0.05);肺癌的不同分化程度间MMP-2的阳性表达差异有统计学意义(P<0.05);肺癌的不同TNM分期间MMP-2的阳性表达差异有统计学意义(P<0.01);生存期<2年者的MMP-2阳性表达率明显高于生存期≥2年者(P<0.01);有与无淋巴结转移的MMP-2阳性表达差异有统计学意义(P<0.01)。结论MMP-2阳性表达上调,MMP-2/TIMP-2的失平衡,MMP-2蛋白过剩,可能是导致肺癌发生的主要原因之一;其可作为肺癌恶性程度及预后判断的指标。 Objective To investigate the expression of matrix metalloproteinase 2 (MMP-2) and tissue inhibitor of metalloproteinase 2(TIMP-2) and their correlation with metastasis and prognosis in human lung cancer. Methods Immunohistochemieal method was used to detect the expression of MMP-2 and TIMP-2 in 60 cases of lung cancer tissues and 20 cases of paracancerous tissues of the lung. Results The positive expression rates of MMP-2 in lung cancer tissues and paracancerous tissues were 73.3%, 15.0 % respectively, whereas the positive rates of TIMP-2 expression in lung cancer tissues and paracancerous tissues were 51.7 %, 10.0 % respectively( P = 0. 000). The positive expression of MMP-2 showed statistically significant differences in different histopathology of lung cancer ( P 〈0.05). The positive expression of MMP-2 showed statistically significant differences in different differentiation of lung cancer ( P 〈0.05). The positive expression of MMP-2 showed statistically significant differences in different TNM stages of lung cancer ( P 〈0.01). Between the survival time ~2 years group and ≥ years groups, the expression of MMP-2 had significant difference ( P 〈0.01). The positive expression of MMP-2 showed statistically significant differences between with metastasis and without metastasis (P〈 0.01). Conclusion Overexpression of MMP-2 and the imbalance of MMP-2 and TIMP-2 are major reasons to result in the lung carcinogenesis . It can act as a reference index in the malignant level of lung carcinoma and prognosis.
