摘要
目的了解血清中可溶性细胞间黏附分子1(sICAM-1)和P选择素(P-sel)对新生儿败血症的诊断意义,并观察两者与病情轻重的相关性。方法新生儿败血症患儿43名,其中23名患儿血培养阳性(病原确诊组),20名血培养阴性(临床诊断组),取同期出生的新生儿15名作为对照组。58例观察对象均抽血检测血清sICAM-1和P-sel,同时进行新生儿危重评分。结果病原确诊组及临床诊断组患儿血清sICAM-1、P-sel明显高于对照组(P<0.01或0.05)。血清sICAM-1、P-sel水平与新生儿危重评分呈高、中度负相关关系(分别是r=-0.780,P<0.01;r=-0.441,P<0.01),血清sICAM-1水平与P-sel水平呈中度正相关关系(r=0.398,P<0.05)。结论新生儿败血症患儿血清中sICAM-1、P-sel水平高于健康新生儿,且与病情轻重呈负相关关系,有望成为诊断新生儿败血症的参考指标。
Objective To study the diagnostic significance of serum soluble intercellular adhesion molecule (sICAM)-1 ,and P-selectin levels in neonatal septicemia. Methods Serum contents of sICAM-1 and P-selectin were detected in 43 cases of neonatal septicemia and 15 normal newborns. Of the 43 cases, there were 23 positive blood cultures and 20 negative ones. All cases were evaluated by critical score for neonates. Results Serum levels of sICAM-1 and P-selectin were significantly higher in septicemia group than those in control group ( P 〈 0.01 or 0.05). Serum levels of sICAM- 1 and P-selectin were negatively correlated with critical score (P〈 0.01 ). There was a positive correlation between serum sICAM-1 and P-selectin contents (P 〈 0.05). Conclusion Serum sICAM-1 and P-selectin contents are related to the illness condition, and may be helpful in the diagncsis of neonatal septicemia.
