自酸蚀与全酸蚀粘结系统用于年轻恒牙早期窝沟龋充填的疗效

摘要 目的:观察在不同粘结(自酸蚀粘结剂或全酸蚀粘结剂)条件下流动树脂充填年轻恒牙早期窝沟龋的疗效。方法:选择60例12~14岁正畸科儿童,双侧均有完全萌出的被诊断为早期窝沟龋的第一(或第二)恒磨牙136颗,采用自身对照方法,每位儿童随机选择一侧磨牙选用自酸蚀粘结剂+流动树脂充填,另一侧选用全酸蚀粘结剂+流动树脂充填。在充填后的3、6、12月采用改良USPHS方法进行临床评价。结果:充填体完整性在12个月时,全酸蚀组明显高于自酸蚀组,P<0.01。边缘密合度、边缘着色、充填后反应、继发龋,两组之间差异无统计学意义,P>0.05。结论:全酸蚀粘结技术流动树脂充填年轻恒牙早期窝沟龋取得较好的临床效果。 Objective:To evaluate the outcomes of restoring early pit and fissure caries in young permanent teeth using flowable filling composites with total-etch and self-etch adhesives. Methods: Sixty children aged 12-14 years old who had early pit and fissure caries in the first or second permanent molars (n=136) were treated. In a self-controlled approach, the tooth on one side was restored with self-etching system and flowable filling composite while the tooth on the other side was restored with total-etching system and flowable filling composite. All patients were followed up and reviewed 3, 6 and 12 months after treatment with modified USPHS criteria. Results: At 12 months, the total-etching group showed remarkably better integrity of the restoration as compared with self-etching group (P〈0.0I). However, marginal adaptation, marginal discoloration, postoperative sensitivity, recurrent caries did not differ between the two groups (P〉0.05). Conclusion: Restoration of early pit and fissure caries in young permanent teeth with total-etching adhesives and flowable filling composites appears to offer better outcomes.
