原发性甲状旁腺功能亢进症25例临床分析

Clinical analysis of 25 cases of primary hyperparathyroidism

目的探讨原发性甲状旁腺功能亢进症(PHPT)的临床特点、误诊原因和治疗效果。方法回顾性分析25例PHP患者的病例资料。结果骨骼系统受累15例,泌尿系统受累9例,无症状1例。22例行病理检查证实甲状旁腺腺瘤17例,甲状旁腺增生4例,甲状旁腺癌1例。结论应提高对PHPT的认识,加强血钙普查,对疑诊者早期检测PTH、颈部B超,确诊者尽早手术。

[Objective] To investigate clinical characteristics of primary hyperparathyroidism and to explore the causes of misdiagnose and treatment of this disease. [Methods] The clinical data of 25 patients was analyzed retro- spectively. [Results] There were 15 patients and 9 patients suffering from skeleton and urinary symptoms respec- tively, and 1 patient with Asymptomatic Primary Hyperparathyroidism. 17 cases was confirmed as parathyroid ade- noma, 4 cases hyperplasia and 1 case carcinoma by pathological examinations. [Conclusion] Blood calcium screening should be emphasized. And for suspected patients, serum PTH measurement and neck ultrasound ought to be applied at an early stage. Parathyroidectomy is the best treatment option for PHPT.
