排痰机对慢性阻塞性肺病血氧饱和度及排痰效果的影响 被引量:2

Effects of medical vibratory sputum excretion machine on saturation of blood oxygen and sputum excretion in patient with chronic obstructive pulmonary disease

摘要 目的探讨医用振动排痰机对慢性阻塞性肺病(COPD)患者血氧饱和度及排痰效果的影响。方法将108例COPD患者随机分为医用振动排痰机组(简称机械组)和人工叩击组(简称人工组),比较2组患者排痰治疗7天前后日排痰量、氧分压(PaO2)、二氧化碳分压(PaCO2)及体温与血常规恢复正常的天数。结果排痰治疗前两组日排痰量、PaO2、PaCO2无明显差异(P>0.05);排痰治疗1周后,两组日排痰量均明显减少、PaO2明显升高、PaCO2明显降低(P<0.01);而机械组与人工组比较,减少日排痰量、PaO2升高和PaCO2降低幅度比人工组更明显(P<0.01);患者体温和血常规恢复正常天数机械组比人工组明显缩短(P<0.01)。结论医用振动排痰机对COPD患者排痰的效果和改善缺氧状态明显优于人工叩击排痰法,具有一定的临床推广应用价值。 Objective To explore the effects of medical vibratory sputum excretion machine on saturation of blood oxygen and sputum excretion in patients with chronic obstructive pulmonary disease (COPD). Methods 108 cases of patients with COPD were randomly divided into two groups with different sputum excretion: medical vibratory sputum excretion machine group (machine group) and artificial beating group (artificial group). The daily quantity of sputum excretion,PaO2, PaCO2 and the recovery time of temperature and blood routine test before and after 7 days of sputum excretion treatment were compared. Results There was no significant difference between two groups in daily quantity of sputum excretion, PaO2 and PaCO2 before treatment (P 〉 0. 05). After one week of sputum excretion treatment, the daily quantity of sputum excretion of two groups was obviously reduced, PaO2 obviously increased, and PaCO2 obviously declined (P〈0. 01). Compared with the artificial group, the reduced quantity of sputum excretion, the increase of PaO2 and decline of PaCO2 were more significant in the machine group(P〈0. 01). The recovery time of temperature and blood routine test of the patients in machine group was obviously shorter than that in the artificial group(P〈0. 01). Conclusion The medical vibratory sputum excretion machine is obviously superior to artificial beating in the effect of sputum excretion and improving hypoxia.
