摘要 目的探讨HIF-1、VEGF在肝门部胆管癌中的表达、临床意义及相互关系。方法用免疫组织化学SP法检测45例肝门部胆管癌及20例非癌胆管组织中HIF-1、VEGF的表达。结果45例肝门部胆管癌中HIF-1、VEGF的表达率分别为68.9%(31/45)和62.2%(28/45),非癌胆管组织中HIF-1、VEGF的表达率分别为0和15%(3/20),差异均有统计学意义(P<0.05)。肝门部胆管癌中HIF-1的表达与淋巴结转移有关(P<0.05),VEGF的表达与肿瘤分化程度和淋巴结转移有关(P<0.05),它们的表达与性别、年龄、肿瘤大小及Bismuth分型无关(P>0.05)。HIF-1、VEGF的表达呈正相关关系(P<0.05)。结论HIF-1、VEGF在肝门部胆管癌中的表达呈正相关关系,共同在肝门部胆管癌的发生、发展、转移中起一定作用,联合检测可判断转移潜能及预后。 Objective: To explore the expression and clinical significance of HIF-1 and VEGF and their correlation in hilar cholangiocarcinoma. Methods : SP method of immunohistochemistry was used to monitor the expressions of HIF-1 and VEGF of 45 cases of hilar cholangiocarcinoma and 20 cases of non hilar cholangiocarcinoma respectively. Results: The rates of expressions of HIF-1 and VEGF were 68.9% (31/45) and 62.2% (28/45) in hilar cholangiocarcinoma respectively, and the rates of expressions of HIF-1 and VEGF were 0 and 15% (3/20) in nonhilar cholangiocarcinoma. There was statistic significance in the comparison ( P 〈 0. 05 ). The expression of HIF-1 was related with lymph node metastasis ( P 〈 0. 05 ) and the expression of VEGF had relationship with tumor differentiation and lymph node metastasis as well ( P 〈 0. 05). The expressions of HIF-1 and VEGF was not related with sex, age, size of tumor and Bismuth Typing (P 〈 0. 05). The expression of HIF-1 had positive correlation with the expression of VEGF ( P 〈 0. 05 ). Conclusion: There is positive correlation between the expression of HIF-1 and the expression of VEGF and they all affect the occurrence, progress and metastasis in hilar cholangiocarcinoma. Joint detection can help to estimate the metastatic potentialities and prognosis.
