漏气音听诊法在左侧双腔支气管导管插管定位中的应用 被引量:2

摘要 目的比较普通听诊法、漏气音听诊法用于左侧双腔支气管导管(L-DLT)插管定位的临床效果。方法60例ASAⅠ-Ⅱ级需行单肺通气(OLV)的患者,按导管插管定位方法分成2组,每组30例。普通听诊法组:麻醉诱导后插入L-DLT,根据两肺分别行OLV时呼吸音的听诊情况调节插管深度定位。漏气音听诊法组:插入L-DLT通过声门后行左支气管腔通气,听诊以L-DLT气管腔漏出的气流声的变化定位。观察记录2组插管定位时间、OLV时气道峰压、临床定位满意率。结果漏气音听诊法定位时间明显短于普通听诊法定位时间,分别为(151.2±44.5)s和(35.1±11.7)s(P<0.01);漏气音听诊法临床定位满意率高于普通听诊法,分别为86.7%和66.7%(P<0.05)。结论漏气音听诊法行左侧双腔支气管插管定位较普通听诊法定位时间短,临床定位满意率高。
