应用组织多普勒Tei指数评价高血压患者左心室功能的研究 被引量:3

摘要 目的:探讨Tei指数评价原发性高血压左室功能的临床价值。方法:选择原发性高血压患者61例,根据左心室重量指数分为两组,无心肌肥厚组(A组)31例,心肌肥厚组(B组)30例,另选30例健康体检者为对照组。以常规超声测量左室内径(LVEDD),舒张期二尖瓣口正向血流峰值比(E/A)与左室射血分数(EF)应用TDI技术于心尖四腔观获取二尖瓣环心肌运动频谱图,分别测量Sm、Em及Am;计算出Em/Am、Tei指数。结果:LVEDD、LVEF及Sm在3组间差异无显著性。Em、Am及Em/Am在A、B两组患者显著降低,尤其以B组明显,A、B组E/A均<1(P<0.05);高血压各组Tei指数比对照组呈递增性增高(P<0.001)。结论:Tei指数能简便、敏感、综合评价高血压左室功能。 Objective;To investigate the clinical value of tissue Doppler Tei index in the evaluation of right ventricular function in patients with hypertension. Methods:Sixty-one patients with mild/moderate hypertension were divided into 2 groups. Thirty-one patients with normal left ventricular mass index(LVMI) were studied in group A, Thirty patients with increased LVMI in group B,and 30 healthy subjects served as control group. The conventional echo-Doppler parameters assessed included the LVEDD,E/A ratio and ejection fraction(EF). The mitral annulus motion was obtained by tissue Doppler imaging(TDI). The peak systolic and early and late diastolic velocities were measured respectively. The Tei index by TDI were calculated. Results :Compared to control group ,the Tei index of RV in hypertension group was significantly increased (P〈0.05). The Em ,Am and Era/Am of the study groups were significantly lower than that of control group,E/A ratio in groups A and B was all less than 1(P〈0.05)The Tei index in groups A and B was all greater than the control group and progressively increased(P〈0. 001 ). Conclusion The Tei index would be valuable and simple for estimating the left ventricular systolic and diastolic functions with high sensitivity in hypertension patients.
