门诊药房从医院剥离的再思考

Perspectives on the Separation of Dispensary for Outpatients from Hospital

目的:探讨将门诊药房从医院剥离出去后对患者、医院及其员工带来的影响。方法:从医疗卫生经费补偿机制、药品流通环节、药学服务功能、患者购药选择权等环节进行思考。结果与结论:在当前医疗卫生机构补偿机制不健全的情况下,药价高企是由于药品流通环节多、定价不合理造成的。盲目剥离门诊药房,不仅使门诊药房监管缺位,药学服务功能丧失,而且患者既不能从中受益,也难以消除不正之风,甚至影响到医疗机构的可持续发展。

OBJECTIVE： To discuss the impact of the separation of dispensary for outpatients from hospital on patients, hospital and its staff. METHODS： The impact of the separation of dispensary for outpatients from hospital were discussed from aspects of hospital reimbursement mechanism of medical funds, medicine flow links, function of pharmaceutical care, patient＇ s rights on drug choice et al. RESULTS ＆ CONCLUSIONS： In view of current unsound reimbursement mechanism, the high drug price was resulted from multiple medicine flow links and irrational pricing of drugs. Blindly separation of dispensary for outpatients from hospital would lead to supervision vacancy of dispensary for outpatients, loss of function of pharmaceutical care, furthermore, patients would not be benefited form the step, and unhealthy tendency （rebate） would be hard to wipe out, more importantly, it would affect the sustainable development of hospital.
