自体刃厚皮与猪脱细胞真皮复合移植的临床应用

CLINICAL APPLICATION OF MESHED PORCINE ACELLULAR DERMIS XENOGRAFT WITH SPLIT-THICKNESS SKIN AUTOGRAFT

目的观察进行自体刃厚皮与猪脱细胞真皮一步法与两步法复合移植修复深度烧伤和瘢痕切除创面的临床效果。方法2004年6月～2006年6月间用自体刃厚皮与猪脱细胞真皮复合移植修复深度烧伤创面或瘢痕切除创面40例,对一步移植法和两步移植法进行比较。结果一步移植法中,10例植皮完全成活,6例植皮95%以上成活,2例植皮80%以上成活;两步移植法中,12例植皮完全成活,10例植皮95%以上成活。均无一例补植皮。随访6个月～2年,一步和两步移植法中的复合移植皮肤外观、质地基本接近正常皮肤,瘢痕增生不明显。结论自体刃厚皮与猪脱细胞真皮复合移植是深度烧伤创面、瘢痕切除创面的良好修复材料,一步移植法可行、简便、实用。

Objective To observe the clinical effects of meshed porcine aeellular dermis xenograft in conjunction with split - thickness skin autograft in the treatment of full - thickness skin burns and the wound after scar resection. Methods 40 patients received the treatment of the skin grat＇ts and the effects of one - and two - stage grafting were compared. Results In those receiving one - stage grafting, 100% survival rate of the grafts was achieved in 10 cases, 95% in 6 cases, and 80% in 2 eases, while in two - stage grafting group, 100% survival rate of the grafts was achieved in 12 eases and more than 95% in 10 cases. No eases needed secondary grafting. During the follow - up of the patients for 6 months to 2 years, we found that the appearance and texture of all the grafts had no significant difference with that of the normal skin. Conclusion The combination of meshed porcine acellular dermis xenograft and split - thickness skin autograft was ideal for repairing full - thickness skin bums, it also shows that one - stage grafting is both simple and practical.
