BacT/Alert 3D系统与改良罗氏培养基分离培养分枝杆菌的效果比较 被引量:4

摘要 目的比较分析BacT/Alert3D全自动分枝杆菌培养系统(3D系统)和改良罗氏培养基(L-J)分离培养分枝杆菌的优缺点。方法将痰液、支气管冲洗液、胸腹腔积液、脑脊液、关节液等标本按送检要求用3D系统或L-J法进行分枝杆菌分离培养,比较结核分枝杆菌(MTB)和非结核分枝杆菌(NTM)分离率,评价分离培养效果;同时对其污染率、培阳报告时间与实验费用进行比较。结果①采用3D系统接种标本2973例,共培养出1020株分枝杆菌,阳性率为34.30%;采用L-J法接种标本7583例,培养出1888株分枝杆菌,阳性率为24.90%。3D系统对MTB的分离率为21.50%,L-J法则为21.10%;3D系统对NTM的分离率为12.80%,L-J法则为3.86%。在各自分离的NTM中,3D系统和L-J法对Ⅰ群(光产色)、Ⅱ群(暗产色)、Ⅲ群(不产色)、Ⅳ群(速生型)分枝杆菌的分离率分别为0.20%和0.06%、3.30%和0.30%、4.40%和1.30%、4.90%和2.20%。②3D系统和L-J法平均报告阳性时间分别是19.30和24.60天;污染率分别是4.00%和2.40%;实验费用是L-J法的5倍左右。结论应用3D系统有助于提高分离培养的阳性率,尤其是对NTM的分离,且分离报告时间较L-J法有较大缩短。但L-J法实验费用低廉,也不需要专门仪器设备,便于普及开展。
