中、老年患者应用丙泊酚联合芬太尼行胃镜检查90例报告

Analysis of propofol combined fentanyl used for painless gastroscopy in 90 middle-aged and elderly patients

目的探讨中、老年患者应用丙泊酚联合芬太尼进行无痛苦胃镜检查的安全性和有效性。方法选取90例自愿接受无痛苦胃镜检查的中、老年患者,按照年龄分为老年组43例和老年前期组47例,术中均给予丙泊酚加芬太尼进行静脉麻醉。比较两组麻醉药物剂量以及检查的安全性和有效性指标。结果所有患者均成功完成胃镜检查,老年前期组患者丙泊酚的剂量大于老年组P<0.05,而芬太尼用量在两组中无差异。两组血压、心率在给药后有一过性下降,但很快自行恢复,且前后比较无统计学差异P>0.05;全程血氧饱和度平稳,给药前后无差异。两组患者均未出现严重不良反应,术后满意度分别为老年组100%和老年前期组97.18%。结论丙泊酚联合小剂量芬太尼应运于中、老年患者胃镜检查是一种舒适、安全、有效的方法。

Objective To study the safety and validity of administration of propofol combined with fentanyl for painless gastroseopic examination in the middle - aged and elderly patients. Methods Included were 90 middle - aged and elderly patients who voluntarily received painless gastroscopic examination. All subjects were divided into the elderly and presenile age group, consisting of 43 and 47 cases respective Both two groups were given intravenous anesthesia with propofol plus fentanyl during gastroscopic examination. Anesthetic dosage and indices of the safety and validity were compared betWeen two groups. Results M1 cases received the gastroscopie examination successfully. The dose ofpropofol was higher in presenile age group than the elderly group P 〈 0.05, while no significant difference was noted in fentanyl dose. A transient reduction in blood pressure and heart rate was observed after administration in both two groups, but returned spontaneously and rapidly to normal transient, demonstrating no marked differences, P 〉 0.05. Andfull -range blood oxygen saturation was stable, without pronounced differences before and after administration,P 〉 0.05. Severe adverse reactions were absent in both two groups. The postoperative satisfaction was 100% in the elderly group and 97.18% in the presenile age group. Conclusions Application of propofol and small - dose fentanyl for gastroscopic examination in the middle -aged and elderly is comfortable, safe andpotent.
