膝关节半月板周缘脱位征象的MRI研究及其临床意义初探 被引量:14

摘要 目的:运用MRI研究膝关节半月板周缘脱位征象,采用切合临床实际的测量方法对其进行研究分析,初步探讨其临床意义。方法:回顾性研究2002年3月-2007年3月间因膝关节症状进行MR检查的539位患者共585个膝关节,结合50例健康志愿者的膝关节,分为骨关节炎组、非骨关节炎有症状组及正常组,分析其半月板脱位发生率。结合年龄、是否伴有骨关节炎等情况,以移位指数作为量化指标进行研究。结果:半月板周缘脱位病例共121例,总发生率为19%。半月板周缘脱位内外侧发生率之比为5.4:1。膝关节骨关节炎者与不符合该诊断者发生率差异有统计学意义(P〈0.01)。所有病例移位指数平均值为0.318±0.091,各年龄组间移位指数的差异具有统计学意义(P〈0.05)。结论:半月板周缘脱位征象的发生率随年龄增长而升高;半月板周缘脱位征象与骨关节炎具有密切联系。 Objective:To describe the phenomenon of radial displacement of the meniscus of the knees with MR imaging by displacement index as well as to inquire into clinical significance of radial displacement of the meniscus .Methods: MR signs of radial displacement of the meniscus were evaluated retrospectively in 539 patients with knee symptoms and 50 cases of asymptomatic controls. According to ACR criteria for knee OA and MRI, the knees were classified as symptomatic knee OA, other symptomatic knee and normal controls. Displacement index that can be refered to age, osteoarthrifis were measured for further analysis. Results: The prevalence of radial displacement of the meniscus was 19%. The ratio between medial and lateral meniscus was 5.4: 1. The incidence of radial displacement for knees with OA signs was greater than that without OA signs. Differences were highly significant ( P 〈 0.01). The average displacement index was 0.318 ± 0. 091. Conclusion:The radial displacement of the meniscus in knees is a common finding in patients with knee symptoms. Radial displacement of meniscus has close connections with age, radiographic signs of osteoarthritis.
