“双固一通”针法对绝经后骨质疏松模型大鼠胰岛素样生长因子-1及组织形态学的影响 被引量:15

The effect of "Shuanggu Yitong" acupuncture therapy on insulin-like growth factor-1 and the bone histomophology of the experimental rats with postmenopausal osteoporosis

摘要 目的探讨"双固一通"针法治疗绝经后骨质疏松症(PMO)的作用机制。方法采用60只6月龄雌性SD大鼠,切除双侧卵巢后饲养90 d,造成实验性骨质疏松症动物模型,随机分成对照组、常规取穴组[6]、"双固"组、"一通"组、"双固一通"组共5组,每组12只。对照组不做任何治疗;常规取穴组取背俞穴"脾俞"、"胃俞"、"肾俞"、"气海俞";"双固"组取"关元"、"足三里";"一通"组取"肾俞"、"膈俞"、"大杼";"双固一通"组取"关元"、"足三里"、"肾俞"、"膈俞"、"大杼"。电针连续治疗12周。①取死大鼠取血清,采用酶联免疫法检测血清胰岛素样生长因子-1;②取同侧股骨上端,制成脱钙骨切片,应用光镜观察骨小梁的形态结构,借助VIDAS图像分析系统测量骨小梁的体积(TBV%)、骨小梁的宽度(Tb.Th)和骨小梁的间隙(Tb.Sp)。结果①与对照组比较,常规取穴组、双固组、一通组的IGF-1均增高(P<0.05),双固一通组显著增高(P<0.01);双固一通组较经验取穴组、双固组、一通组显著增高(P<0.05);经验取穴组、双固组、一通组3组之间无显著性差异(P>0.05);②与对照组比较,所有治疗组单位面积内骨小梁的体积及宽度增大(P<0.05),间隙变窄(P<0.05),双固一通组骨小梁的体积及宽度增大显著(P<0.01),间隙变窄显著(P<0.01);双固一通组较常规取穴组、双固组、一通组的骨小梁的体积和宽度增大(P<0.05),间隙变窄(P<0.05);常规取穴组、双固组、一通组3组之间比较无统计学意义(P>0.05)。结论①针刺能够提高去卵巢骨质疏松症模型大鼠IGF-1的水平,且"双固一通"针法优于常规取穴针法;②针刺在一定程度上可以改善或恢复绝经后骨质疏松症模型大鼠的骨组织形态结构,"双固一通"针法优于其他常规取穴针法。 Objective To study the mechanisms that acupuncture therapy of "Shuanggu Yitong" can treat postmenopausal osteoporosis (PMO). Methods Choosing female SD rats aged 6 months then resecting double ovaries and feeding them for 90 days in order to make the model of experimental osteoporosis. Then, dividing them into 5 groups such as control group, regular acupoints applying group, Shuanggu group, Yitong group, Shuanggu Yitong group, with 12 rats each group. There wasn't any therapy in control group; We applied Pishu(BL 20), Weishu( BL 21 ), Shenshu( BL 23 ), Qihaishu ( BL 24) for the regular acupoints applying group ; Guanyuan ( RN 4), Zusanli(ST 36) for the Shuanggu group; and Shenshu(BL 23), Geshu(BL 17), Dazhu(BL 11) for the Yitong group ; Guanyuan( RN 4), Zusanli( ST 36), Shenshu ( BL 23 ), Geshu ( BL 17 ), Dazhu ( BL 11 ) for the Shuanggu Yitong group. Giving clectroacupuncture therapy once a day, 15 minutes each time, holding the needles for 10 minutes. Then these rats rested for 1 day after consecutive therapy for 6 days. The therapy lasted for 12 weeks.(1) Applying ELISA to examine insulin-like growth factor-1 (IGF-1). (2) Using microscope to observe the morphological structure of bone trabeculae, applying VIDAS image analyzer system to measure % trabecular volume (TBV % ), trabecular thickness (Tb. Th) and trabecular separation (Tb. Sp). Results( 1 ) Compared with control group, in the other four groups, IGF-1 all increased ( P 〈 0.05), IGF-1 increased significantly in Shuanggu Yitong group ( P 〈 0.01 ) ; Compared with regular acupoints applying group, Shuanggu group, Yitong group, IGF- 1 increased in Shuanggu Yitoug group (P 〈 0.05); There are no statistic significance among regular aeupoints applying group, Shuanggu group, Yitong group ( P 〉 0.05). (2)Compared with control group, the volume and thickness of bone trabeculae in per stable area enlarged for all the therapy groups ( P 〈 0.05 ), the separation got narrow ( P 〈 0.05), the volume and thickness of bone trabeculae enlarged significantly in Shuanggu Yitiong group (P 〈 0.01 ), separation narrowed significantly ( P 〈 0.01 ) ; Compared with regular acupoints applying group, Shuanggu group, Yitong group, the volume and thickness of bone trabeculae in Shuanggu Yitiong group enlarged (P 〈 0.05), separation got narrow ( P 〈 0.05) ; There are no statistic significance among regular acupoints applying group, Shuanggu group, Yitong group ( P 〉 0.05). Conclusion ( 1 ) Acupuncture can increase IGF-1 of osteoporosis model rats without ovaries and "Shuanggu Yitong" acupuncture therapy is superior to other regular acupoints in applying acupuncture therapy. (2)Acupuncture can improve or recover bone histomophology of postmenopausal osteoporosis model rats in a way and "Shuanggu Yitong" acupuncture therapy is superior to other regular acupoints in applying acupuncture therapy.
