摘要
观察转移相关黏附分子拼接变异体CD44v6在腹水胃癌细胞中的表达率,评价CD44v6是否作为腹水胃癌细胞特异性标志物,探讨胃癌细胞的转移机理。用TM区分腹水中胃癌细胞与间皮细胞,并与M elano-cyte/MC进行了敏感性、特异性对比。从腹水液基薄层细胞涂片中筛查出53例胃癌细胞,28例间皮细胞进行常规细胞石蜡包埋切片,采用鼠抗人CD44v6,TM,MC特异性抗体,对53例胃癌细胞,28例间皮细胞进行了免疫组化检测,应用χ2检验。CD44v6在腹水胃癌细胞中的检出率为96.2%(51/53),间皮细胞中的检出率为7.14%(2/28);TM,MC在间皮细胞中的检出率分别为96.4%(27/28),92.9%(26/28),胃癌细胞中的检出率为7.55%(4/53)22.6%(12/53);TM,MC对间皮细胞敏感性分别为96.4%(27/28)、92.9%(26/28);TM,MC对间皮细胞特异性为92.5%(49/53)、77.4%(41/53)。薄层细胞涂片、石蜡包埋技术能够有效地开展细胞免疫化学检测,并且具有广阔发展前景。CD44v6在胃癌细胞中的检出率明显高于间皮细胞,提示参与了胃癌细胞的浸润转移,可作为胃癌细胞的标志物。TM对间皮细胞检测的敏感性、特异性要高于MC。CD44v6协同TM对于胃癌细胞和间皮细胞的鉴别诊断有重要的意义。
Differential expressions of human metastasis molecule adhesion CD44 splice variant 6 (CD44v6) in gastric carcinomas (GC) was investigated and to evaluated whether CD44v6 could be a useful specific marker in ascites. TM was used to distinguish GC cells and mesothelium cells, and to compare the sensitivity and specificity of Thrombodu- lin/TM with melanocyte/MC. 53 cases of GC cells and 28 cases of mesothelium cells were sieve examined through thin ascites base smear and carried out routine paraffin embedding slice, then adopted mAb against CD44v6, TM, MC to carry out immunohistochemical tests on the 53 cases of GC cells and 28 cases of mesothelium cells and examined with x2. The results showed that the detection rate of CD44v6 in GC cells in ascites was 96.2% (51/53), and 7.14% (2/28) (P 〈0.05) in mesothelium cells. The detection rate of TM and MC in mesothelium cells in ascites were 96.4% ( 17/28), 92.9% (26/28) , 7.55% (4/53), 22.6% ( 12/53 ) in GC cells. The sensitivity of TM and MC in mesothelium cells were 96.4% (27/28) , 92.6% (26/28) ; Specificity of TM and MC in GC cells were 92.5% (49/53) , 77.4% (41/53). Therefore, thin ascites base smear and paraffin embedding technique could effectively develop immunochemical inspection and posseses broad develo- ping prospect. The detection rate of CD44v6 in GC cells was obviously higher than in mesothelium cells suggested that it participated infiltration metastasis of GC ceils and can be used as GC cells' marker. The sensitivity and specificity of TM to mesothelium cells were higher than MC, therefore the coordination of CD44v6 with TM has great significance in differential diagnosis of GC cells and mesothelium cells.
