立体心电图对陈旧心肌梗死的诊断价值

The value of three-dimensional electrocardiogram on old myocardial infarction

目的评价立体心电图(three-dimensional electrocardiogram,3D-ECG)对陈旧心肌梗死的诊断价值。方法入选陈旧心肌梗死患者212例,非心肌梗死患者88例作为对照组,顺序应用心电图机和立体心电图仪记录12导心电图(electrocardiogram,ECG)及立体心电图,计算各自诊断的敏感性、特异性和准确性。结果立体心电图和心电图对陈旧心肌梗死的诊断敏感性分别为99.06%和62.26%、准确性分别为98.00%和72.00%,差异有显著性(p<0.01);特异性均为95.45%,无显著差别。结论立体心电图是一种良好的检查手段,对陈旧心肌梗死有较高的诊断价值,可减少误诊和漏诊率。

Objective To evaluate the value of three-dimensional electrocardiogram on old myocardial infarction. Methods select 212 old myocardial infarction patients （group A） and 88 non-myocardial infarction （group B）, record 12 lead electrocardiogram and three- dimensional electrocardiogram in sequence, calculate their sensitivity, specificity and accuracy respectively. Results The sensitivity and accuracy of three-dimensional electrocardiogram and electrocardiogram are 99.06% vs 62.26%, 98.00% vs 72.00% respectively, between which difference was significant （p〈0.01）. Both specificity are 95.45%, no significant difference between them. Conclusion Three-dimensional electrocardiogram is superior than electrocardiogram on the diagnose of old myocardial infarction and can decrease the rate of misdiagnosis and the rate of missed diagnosis.
