骨桥蛋白及血管内皮细胞生长因子与类风湿关节炎滑膜血管发生的关联 被引量:6

Correlation of Osteopontin and Vascular Endothelial Growth Factor with Angiogenesis in Rheumatoid Arthritis

摘要 目的研究骨桥蛋白(OPN)、血管内皮细胞生长因子(VEGF)在类风湿关节炎(RA)患者滑膜组织的表达及其与滑膜血管形成的关系。方法应用免疫组织化学技术检测7例类风湿关节炎、5例骨关节炎(OA)和1例外伤后患者关节滑膜组织的 OPN 及 VEGF.计数 OPN、VEGF 阳性细胞数及血管数目,比较它们之间的相关性。结果 RA 滑膜衬里层和下层 OPN 阳性细胞数与滑膜血管数量呈正相关(r=0.837,P=0.019;r=0.946,P=0.010)。OA 滑膜衬里层和下层 OPN 阳性细胞数与血管数量无相关性(r=0.872,P=0.054;r=0.154,P=0.805)。RA 滑膜下层 VEGF 阳性细胞数和血管数目呈正相关(r=0.982,P=0.010)。RA 滑膜衬里层及下层 OPN 阳性细胞数和相应滑膜层 VEGF 阳性细胞数呈正相关(r=0.821,P=0.023;r=0.964,P=0.010);OA 滑膜衬里层及下层 OPN阳性细胞数和相应滑膜层 VEGF 阳性细胞数无相关性(r=0.700,P=0.188;r=0.500,P=0.391)。结论 OPN 在RA 的滑膜血管形成中起重要作用,OPN 和 VEGF 可能协同作用参与 RA 滑膜血管翳生成及软骨破坏。 Objective To evaluate the expressions of osteopontin (OPN) and vascular endothelial growth factor (VEGF) in the synovium of rheumatoid arthritis (RA) patients, and its possible correlation with synovial angiogenesis. Methods Immunohistochemistry was used to detect OPN and VEGF expressions in synovial tissue of 7 patients with RA, 5 patients with osteoarthritis (OA), and 1 patient with joint trauma. The number of OPN and VEGF positive cells and vessels were counted. The correlation of OPN or VEGF expression with vessel numbers were analyzed. Results OPN expression in the lining and sublining layer was significantly correlated with the number of vessels in the same layer of synovium (r=0.837, P=0.019; r=0.946, P= 0.010). No significant correlation was found between OPN expression and vessel number in OA patients (r = 0.872, P= 0.054; r = 0.154, P= 0.805). In RA, VEGF expression in the sublining layer was significantly correlated with vessel number (r = 0.982, P = 0.010). OPN expression in the lining and sublining layer was significantly correlated with VEGF expression (r = 0.821, P = 0.023 ; r = 0.964, P =0.010). However, OPN and VEGF expressions did not reach statistical significance (r = 0.700, P= 0.188; r = 0.500, P = 0.391 ) in OA patients. Conclusion OPN is very important in the angiogenesis of RA. OPN and VEGF are synergistical in the formation of synovial pannus and the destruction of cartilage.
