射频热凝毁损治疗三叉神经痛

Treatment of trigeminal neuralgia with neurolytic radiofrequency thermocoagulation

目的探讨经皮穿刺射频温控热凝治疗三叉神经痛的疗效。方法对住院的105例三叉神经痛患者采用经皮穿刺半月神经节或侧人路行上颌神经、下颌神经射频温控热凝毁损治疗,温度用60℃、65℃、70℃、75℃、80℃、85℃各1次,每次60s。结果105例中,Ⅰ支12例,Ⅱ支45例,Ⅲ支23例,Ⅱ、Ⅲ支同时受累25例。治疗后3个月评定疗效.优效84例(80.0%),良效15例(14.2%),3例经2次治疗后获良效(2.9%);无效者3例(2.9%),包括虽经2次治疗仍有部分疼痛者。总有效率为97.1%。除原疼痛部位有不同程度的感觉减退外,无其他严重、永久并发症。结论采用经皮穿刺射频温控热凝治疗三叉神经痛操作安全,治疗效果良好。

Objective To observe the effect of transcutaneous radiofrequency thermocoagulation for treating trigeminal neuralgia . Methods One hundred and five inpatients with trigeminal neuralgia were treated with neurolytic radiofrequency thermoeoagulation to the ganglion semilunare, infraorbital foramen, maxillary nerve, inferior maxillary nerve. Temperature of 60℃, 65℃. 70℃, 75℃, 80℃, 85℃ were use in turns . 60 s per time. Results Of 105 cases, there were 19 cases in branch Ⅰ. 45 cases in branch Ⅱ . 23 cases in branch Ⅲ , and 25 cases in branches Ⅱ ＋ Ⅲ. The results showed that excellent effect was 84 cases （80.0%）, good was 15 cases （14.2%）. including 3 cases （2.9%） who were treated two times, ineffective 3 cases （2.9%）. The total effective rate was 97.1%. There were no severe . permanent complications except hypoesthesia on the primary area in different level. Conclusion The treatment of trigeminai neuralgia with radiofrequency thermocoagulation is an effective and safety method.
