中西药结合治疗妊娠期外阴阴道念珠菌病临床效果观察

Clinical Effect Survey on the Treatment of Vulvovaginal Candidiasis during Pregnancy with Combined Traditional Chinese and Modern Medicine

目的:观察中西药结合治疗妊娠期外阴阴道念珠菌病的临床效果。方法:将109例妊娠期外阴阴道念珠菌病患者统一排序入组,中西药组采用中药坐浴+克霉唑栓+乳杆菌活菌胶囊治疗;中药组采用采用中药坐浴+乳杆菌活菌胶囊治疗;西药组采用克霉唑栓+乳杆菌活菌胶囊治疗。观察3组病例治疗前、治疗后7天、1个月、2个月,各组症状、体征和镜检指标情况。结果:①治疗后白带量增多、白带性状、外阴瘙痒、外阴充血、外阴红肿、阴道充血,以及病原体转阴,3组间差异有统计学意义,中西药组和西药组于治疗后7天优于中药组。治疗后1个月、2个月,中西药组复发率低于中药组和西药组。②症状、体征、实验室检查结果在治疗后7天、1个月、2个月均有不同程度的好转,在不同随访时间差异有统计学意义。各时点与组间无交互作用。③外阴灼痛、尿频、尿痛、皮肤抓痕或皲裂、阴道黏膜浅表糜烂和溃疡、阴道清洁度于治疗后组间差异无统计学意义。结论:以生态疗法为基础的中西药结合治疗方案(中药坐浴+克霉唑栓+乳杆菌活菌胶囊)更能明显改善症状和体征,降低复发率,是一种治疗妊娠期外阴阴道念珠菌病的安全、有效的方法。

Objective： To investigate the clinical effects of the combined traditional Chinese and modem medicine treatment of vulvovuginal candidiasis （vvc） in pregnant women. Methods： 109 patients with vulvovaginal candidiasis were included in this study, and divided into three groups randomly. The patients of the combined traditional Chinese and modem medicine group received therapeutic alliance of traditional Chinese medicine demibain, clotrimazole plug, and lactobacilli capsule; the traditional Chinese medicine group with herb demibain and lactobacilli capsule; and the modem medicine group with clotrimazole plug and lactobaciUi capsule. The occurrence of clinical syndromes, signs, and eolpescopy examination indexes were observed among the three groups, pre - and pest - therapy （the seventh day, the first and second month after treatment）. Results： ① There were statistical differences in the following aspects among the three groups： leukorrheal amount and quale, vulval itching, congestion, swelling, vaginal congestion, and the turning negative of pathogens. The clinical effects of the combined medicine group and the modem medicine group were better than the traditional medicine group at the 7th day after treatment, and the recurrent rate of vulvovaginal candidiasis of the combined medicine group was lower than the other two groups, during the first and second month post - therapy. ② All clinical syndromes, signs, and laboratory examinations indexes were improved after treatment, and the results showed statistical differences among each different observation time. Even each time and group appeared no interaction. ③ However, there were no statistical differences between pre - and pest - therapy in these indexes, including： vulval eausalgia, urinary frequency, odynuria, scratching and chapping, vaginal mucesa erosio and ulcer. Conclusion： Compared with traditional Chinese medicine and modem medicine, the combined traditional Chinese and modern medicine treatment can improve the clinical signs and syndromes, and decrease the recurrent rate. The results show that this treatment is a safe and effective method for vulvovaginal candidiasis during pregnancy.
