静脉负荷剂量胺碘酮治疗心力衰竭并阵发性心房颤动疗效观察

Efficacy Observation of Intravenously Administer Loading-Dosage of Amiodarone for Chronic Heart Failure with Paroxysmal Atrial Fibrillation

目的:探讨静脉负荷剂量胺碘酮治疗心衰并阵发性房颤的临床效果及安全性。方法:将64例心衰并阵发性房颤患者随机分为治疗组和安慰剂组。治疗组患者1 h内先静脉注入胺碘酮300?,然后20.-1静滴维持24 h。安慰剂组则给生理盐水静注并维持24 h。观察房颤转复情况、心功能、血压和心室率变化及不良反应。结果:治疗组总有效率优于安慰剂组(P<0.01),治疗组平均转复时间明显短于安慰剂组(P<0.01),治疗组心室率明显低于安慰剂组(P<0.01)。结论:胺碘酮是治疗心衰并房颤安全、有效的复律药物。

Objective： To assess the efficacy and safety of intravenously administer loading- dosage of amiodarone for chronic heart failure （CHF）with paroxysmal atrial fibrillation（Af）. Methods：Sixty four patients suffering from CHF with paroxysmal Af were randomly divided into treatment group or placebo group. The treatment group was firstly treated with intravenous injection of 300 mg of amiodarone within 1 hr, and then intravenous transfusion of 20 mg·kg^-1 continuously for 24 h, while the placebo group was adeministered normal saline and maintained for 24 h. The advert effect of amiodarone, the chang of heart rate , blood pressure and cardiac function, and the reversion of Af were observed. Results： The total efficacy rate of treatment group was higher than that of placebo group（P 〈 0.01 ）. The mean reversion times of the two groups were mobvious difference（ P 〈 0.01 ）. The mean ventricular rate of treatment group was apparent lower than placebo group（P 〈 0.01 ）. Conclusion： Amiodarone is safe and effective for CHF with Af.
