摘要
[目的]探讨结肠癌并肠梗阻的诊断与外科手术治疗方法。[方法]回顾性分析某院1990年1月～2006年12月54例老年结肠癌并肠梗阻病人的外科治疗资料。[结果]54例中I期切除46例,II期切除6例,单纯结肠造瘘2例.术后并发症12例(22.2%).本组病人无手术死亡。[结论]手术切除是高龄结肠癌病人治疗的良好方法,术前尽快明确诊断、缓解梗阻、改善全身状态,争取行I期手术。充分的肠道准备,适当的麻醉和手术方式,有效的预防和治疗并发症是提高疗效的关键。
[Objective] To explore appropriate diagnosis and treatment methods for aged colonic cancer patients complicated with obstruction. [Methods] Retrospectively analyzed treatment data of 54 aged colonic cancer patients complicated with obstruction from Jan.1990 to Dec.2006. [ Results] Among the 38 patients, primary excision was done in 46 patients, secondary excision in 6, and simple colostomy fistula in 2. Postoperative diseases were found in 12 cases （22.2%）. There was no operation related decease. [Conclusion] Operative excision is an effective treatment for aged colonic cancer patients. It is neeessary to clarify the diagnosis, relieve obstruction and improve general conditions as soon as possible. Sufficient intestinal preoperative preparation, reasonable choices of anaesthetic and operative methods are the keys for effective prevention and treatment of postoperative complications.
