IGF-Ⅱ,IGF-Ⅰ R在结肠腺瘤中的表达及其生物学意义 被引量:2

Expression and biological significance of IGF-Ⅱ and IGF-ⅠR in colorectal adenoma

摘要 目的:观察IGF-Ⅱ,IGF-ⅠR在结肠腺瘤中的表达,探讨其生物学意义。方法:免疫组化方法分别检测IGF-Ⅱ,IGF-ⅠR,PCNA在正常结肠黏膜组(A组n=20)、低级别上皮内瘤变组(B组n=50)、高级别上皮内瘤变组(C组n=50)和结肠癌组(D组n=20)中的表达,TUNEL法检测细胞凋亡。结果:IGF-Ⅱ,IGF-ⅠR,PCNA在结肠癌组和高级别上皮内瘤变组中呈高表达,两者之间无显著差异(P>0.05),分别与低级别上皮内瘤变组、正常大肠黏膜组相比表达有显著差异(P<0.01);低级别上皮内瘤变组、高级别上皮内瘤变组、结肠癌组的凋亡指数(apoptotic index,AI)逐渐降低,且两两比较有显著差异(P<0.01)。IGF-Ⅱ,IGF-ⅠR的表达与增殖指数(proliferative index,PI)成正相关、与AI成负相关;PI,AI在低级别上皮内瘤变组IGF-Ⅱ,IGF-ⅠR阴性和阳性中无显著差异(P>0.05),在高级别上皮内瘤变组则有显著差异(P<0.01)。结论:IGF-Ⅱ,IGF-ⅠR的异常表达与结肠腺瘤癌变密切相关;IGF-Ⅱ,IGF-ⅠR促进细胞异常增殖,抑制肿瘤细胞凋亡,在结肠腺瘤癌变过程中起了重要的作用。 Objective : To observe the expression of IGF- Ⅱ and IGF-Ⅰ R in colorectal adenoma and investigate its biologial Significance. Methods : 140 patients were divided into four groups :20 cases of normal colon mucosa( A group), 50 cases of low grade colon intraepithelial neoplasia (B group),50 cases of high grade colon intraepithelial neoplasia( C group) and 20 cases of colorectal carcinoma( D group). The expressions of IGF-Ⅱ , IGF-Ⅰ R, PCNA and apoptosis were detected by immunohistochemical and TUNEL. Resuits: The expressions of IGF-Ⅱ , IGF-Ⅰ R and PCNA in C and D grOuP was higher than A and B group and there was no significant difference between C and D group(P 〉 0.05 ) ; compared with A and B group there were significant differences (P 〈 0. 01 ); AI( Apoptotic index) in adenomas and adenocarcinomas of adenoma-carcinoma sequence was gradually less(P 〈 0.01). The expressions of IGF-Ⅱ , IGF-Ⅰ R had positive correlation with the expression of PI( proliferative index) and negative correlations with the expression of AI. The expression of PI ,AI in B group had no significant difference between the positive and negative expression of IGF-Ⅱ ,IGF-Ⅰ R( P 〉 0.05 ), but in D group had significant difference ( P 〈 0. 01 ). Conclusion: Abnormal expressions of IGF-Ⅱ ,IGF-Ⅰ R have intently relations from colorectal adenoma to carcinoma ; IGF-Ⅱ and IGF-ⅠR can promote cell abnormal proliferation and inhibit tumor cell apoptosis. And have an important effect on the process from cotorectal adenoma to carcinoma.
