统计参数图在脑血氧水平依赖功能磁共振成像研究中的应用评价

摘要 目的:探讨统计参数图在脑血氧水平依赖功能磁共振成像研究中的应用价值。方法:本研究被试者共87例,其中中枢性运动功能异常组被试57例,正常对照组被试30例。本研究的运动功能刺激实验设计为组块设计(Block-Design),激活数据采集在双梯度1.5TMR扫描仪上完成,包括定位像、解剖像、功能像、全脑3D像。所有数据用统计参数图软件包离线后处理,经过一系列的预处理和统计分析,得到脑运动区激活统计参数图,研究脑激活区与脑解剖功能区的对应关系,并生成各种量化分析数据以进行相关性分析等统计学处理。结果:异常组被试57例脑血氧水平依赖功能磁共振成像原始资料,经统计参数图后处理,结果符合临床诊断要求的46例(80.70%),正常对照组被试30例,经统计参数图后处理,结果符合临床诊断要求的29例(96.60%),两组结果比较相差显著(X2=4.21,P<0.01),提示正常对照组被试数据经统计参数图后处理结果符合诊断要求的百分率大于异常被试组。结论:统计参数图内置多项预处理步骤,可较有效地排除或减弱被试者的头动、心跳、呼吸及MRI设备系统噪声等干扰因素造成的伪影/影响,有效的预处理在肢体运动功能异常者的脑血氧水平依赖功能磁共振成像数据处理中起了至关重要的作用,是确保脑血氧水平依赖功能磁共振成像激活结果的灵敏性和可靠性的关键手段。 Objective: To explore the value of SPM2 in study of Bold - fMRI of motor - related cerebral dysfunction. Method: Fifty - seven patients, who were suffered from motor- related dysfunction with different kinds of fectors, served as experimental group. Thirty healthy right - handed persons matched in age and gender were recruited and served as control group. Block - Design was used as the mission in the study of 2 groups . All of the subjects were given Bold- fMRI examination with GE 1.5T Exdte HD MRI system. Positional images, anatomic images, the whole brain three - dimensional images, and functional images were collected. Data processing of Bold- fMRI were completed off- line with SPM2 software. The sizes and signal intensities of activation regions of brain were measured, the results of two groups were compared. Results: All of Fifty- seven patients with motor- related cortex dysfunction and thirty controls participated in the final result analysis. 46 eases (80.70%) of 57 patients accorded with the demands of clinical diagnoses. Twenty - nine (96.60%) persons in normal control group accorded with clinical diagnoses. The results of both groups were significant (X^2 = 4.21, p〈=0.01) . Concluson: SPM2 can effectively corrected the movement of head, eliminated the noise causing by heartbeat, breath, and MR systemic, through a series of pretreatment. It played an important part in data processing and analysis in the study of Bold - fMRI of motor related cortex dysfunction, and ensured the reliability of the results and quality of Bold - fMRI.
