小儿口服与静脉给药血清地高辛浓度及中毒反应

摘要 测得86例心衰小儿血清地高辛浓度治疗范围(1.9±0.61)μg/L,不同年龄组的治疗浓度无明显差异。20例口服与静脉给药对比,当静脉给药剂量为口服给药的2/3时,前者的血药浓度明显高于后者。血药浓度≥2μg/L时,地高辛中毒的发生率明显增加,且均为心肌疾病患儿。治疗过程中出现心律紊乱或消化道症状不能肯定为地高辛中毒。 The range of therapeutic serum concentrations of digoxin was (1. 19 ± 0. 61)μg/L obtained in 86 children with heart failure. There was no significant difference in the therapeutic serum levels of digo- xin between different age groups. Serum levels of digoxin injected intravenously were significantly higher than those of digoxin given orally when the Ⅳ dosage was 2/3 of the oral dosage. Incidence of digoxin intoxication in children with myocardial disease increased markedly when serum concentration of digoxin was ≥2μg/L. The occurrence of arrhythmia and digestive tract symptoms could not confirm the diagnosis of digoxin intoxication if there was no confirmative serum digoxin levels tested.
