类胰岛素生长因子-1在先天性马蹄内翻足中的临床意义

摘要 目的探讨先天性马蹄内翻足血清中类胰岛素生长因子-1(IGF-1)的变化及临床意义。方法采用酶联免疫吸附法检测49例先天性马蹄内翻足患儿(双足畸形23例、单足畸形26例)和18例排除与IGF-1有关疾病的外科住院患儿血清IGF-1水平,并随访2年,观察所有正常足和畸形足的发育情况。结果23例双足马蹄内翻足患儿,14例双足发育(足底长)在正常范围,其平均血清IGF-1水平与对照组比较,差异无统计学意义(P>0.05);9例存在不同程度的双足发育落后,其平均血清IGF-1水平与对照组比较,差异有统计学意义(P<0.05);26例单足马蹄内翻足患儿,17例患足发育正常,9例患足发育落后,但两者平均血清IGF-1水平与对照组比较,差异均无统计学意义(均P>0.05)。结论双侧先天性马蹄内翻足患儿血清中IGF-1水平的变化可以作为评估患足发育情况的一个辅助指标。 Objective To evaluate the clinical significance of IGF-1 in congenital club foot. Methods Fourty-nine patients have been followed for two years and eighteen negative controls .The serum IGF-1 concentration is determined using Enzyme Linked Immunosorbent Assay (ELISA)before the operation and the length of all foot is determined after two years. Results The IGF-1 concentration is analysed with SPSS software. We find that serum IGF-1 concentration in congenital both club feet with hypogenesis of feet is significantly lower than that of both club feet with normal foot and the negative control, while the difference between the single club foot with hypogenesis of foot and the negative control was not significant.Conclusion Examining serum IGF-1 concentration before operation by ELISA technique is a useful method to elaverate the development of foot in the congenital both club feet and not single club foot.
