摘要
目的:通过对缺血性脑卒中急性期TCD观察,并于CT进行比较,以探讨西宁地区TCD对缺血性脑卒中早期诊断价值。方法:对50例首次发病住院患者于24h内进行床旁TCD检查及CT检查,门诊健康体检者为对照组,检查结果进行统计学比较。结果:脑卒中组TCD检测结果,病侧MCAVm降低、ACAVm升高,分别与自身健侧及对照组比较差别有显著性(P<0.01)。病例组TCD和CT两种检测结果进行X2检验,X2=39.208,P<0.001,差别有显著性。结论:在西宁地区缺血性脑卒中急性期TCD异常率高于CT,具有较高的早期诊断价值。
Objective:To assess the effect of trans-cranial Doppler (TCD) on early diagnosis of ischemic cerebral stroke at acute stage, as comparing with the CT imaging, in Xining area. Methods. 50 patients who hospitalized with primary ischemic cerebral stroke were assigned within 24 hours after administration to perform beside TCD and CT examination as well as in the 52 healthy control subjects who did a physical examination of TCD and CT in the outpatients. The results of examination would be compared be- tween both cohorts. The average flow velocity (Vm) of the bilateral middle cerebral artery (MCA), ante -cerebral artery (ACA), post-cerebral artery (PCA) the bilateral vertebral artery (VA) and the basilar artery (BA) were recorded by TCD. Results.The MCAVm is decreased at event side of brain, as com- pared with the controls and at normal side, and there was a significant difference, in contrast in the ACAVm (P〈0. 01). The sensitivity of TCD test is 88 per cent. CT sensitivity is 26 per cent(X^2 test, ;X^2 = 39. 208, P〈0. 001), Conclusions: The accuracy rate of TCD is higher in diagnosis of event of cerebral stroke at acute stage than that CT, this suggests TCD is a better diagnostic way for early ischemic cerebral stroke in Xining area.
