人脑胶质瘤MMP-9表达水平与白质纤维束损害程度的相关性研究

摘要 目的探讨人脑胶质瘤MMP-9表达水平与白质纤维束损害程度之间的相关性。方法分别采用磁共振扩散张量成像和免疫组织化学染色方法检测脑胶质瘤患者手术前白质纤维束损害程度和手术后肿瘤细胞MMP-9表达水平的变化,并分析二者之间的相关性。结果低级别胶质瘤组患者白质纤维束损害程度轻微,以邻近和(或)推移为主(84.00%,21/25);MMP-9呈低水平表达(52.00%,13/25),其表达水平与白质纤维束损害程度呈负相关(rs=-0.831,P=0.036)。高级别胶质瘤组患者白质纤维束损害程度严重,以浸润和(或)破坏为主(83.33%,15/18),MMP-9呈高表达并与白质纤维束损害程度呈正相关(rs=0.763,P=0.037),两级别组之间差异有统计学意义(P=0.000)。结论脑胶质瘤恶性程度越高,对白质纤维束的损害程度越严重,肿瘤细胞MMP-9表达水平越高;二者的相关性研究可为脑胶质瘤的序贯治疗提供参考依据。 Objective To explore the correlation between the expression level of MMP-9 and the damage extent of lacertus in human cerebral glioma. Methods The damage extent of lacertus and the expression level of MMP-9 in human cerebral glioma were examined by diffusion tensor magnetic resonance imaging (DTI) and immunohistochemistry, respectively, and the correlation between them was analysed. Results In low grade glioma group, the damage of lacertus was slight and tumors was mainly close to and/or advanced to the lacertns (84.00%, 21/25), and the expression level of MMP-9 was rather low (52.00%, 13/ 25). The correlation between the expression level of MMP-9 and the damage extent of lacertus was negative (rs=-0.831, P=0.036). In high grade glioma group, the damage of lacertus was severe and was mainly to infiltrated and/or destroyed (83.33%, 15/18), and MMP-9 was highly expressed. The expression level of MMP-9 and the damage extent of lacertus presented positive correlation (rs = 0.763, P = 0.037). There was significant difference between low and high grade glioma group (P=0.000). Conclusion The damage extent of lacertus by glioma appears to correlate with the malignancy of glioma and the expression level of MMP-9 in glioma. The correlation is considered as a basis of the sequential therapy of glioma.
