知柏前清汤联合左旋氧氟沙星治疗对慢性前列腺炎患者精液质量的影响

Influence of Zhibai Qianqing Decoction Combined with Levofloxacin on Sperm Quality of Chronic Prostatitis Patients

目的观察知柏前清汤联合左旋氧氟沙星治疗对慢性前列腺炎患者精液质量的影响,并从精子凋亡角度初步探讨其可能的机制。方法慢性前列腺炎患者58例,治疗前行精液分析、精子凋亡率检测并与健康组进行比较。将患者随机分为治疗组(28例)和对照组(30例),治疗组用知柏前清汤联合左旋氧氟沙星治疗,对照组仅用左旋氧氟沙星治疗。2组均在治疗1个月后对比观察临床疗效,精液分析,并采用TUNEL法分别检测治疗后精子凋亡率的变化。结果慢性前列腺炎患者与健康组比较,精液液化时间延长(P<0.01),精子活力明显下降(P<0.01)。治疗组治疗后液化时间明显缩短(P<0.01),精子活力与治疗前相比有显著提高(P<0.01)。对照组治疗后液化时间明显缩短(P<0.05),但精子活力改变差异无统计学意义(P>0.05)。慢性前列腺炎患者精子凋亡率显著高于健康组(P<0.01)。治疗后治疗组精子凋亡率较治疗前明显降低(P<0.01)。对照组治疗前后精子凋亡率变化差异无统计学意义(P>0.05)。治疗组临床治愈率35.71%,有效率85.71%,优于对照组(P<0.05)。结论知柏前清汤联合左旋氧氟沙星治疗能有效改善慢性前列腺炎患者的精液质量,这种作用可能是通过降低精子凋亡率实现的。

Objective To observe the influence of Zhibai Qianqing （ZBQQ） Decoction combined with Levofloxacin on sperm quality of chronic prostatitis （CP） patients and explore its possible mechanism through sperm apoptosis rate. Methods Fifty eight cases were divided into 2 groups randomly. Twenty eight cases were served with ZBQQ decoction combined with Levofloxacin （observe group）, 30 cases were treated by Levofloxacin tablets （control group）. The curative effect of the two groups was compared one month later. All the cases underwent semen analysis. Sperm apoptosis rates were measured by TUNEL before and after the treatment. Results Sperm liquefaction time of CP group was significantly longer than the normal group （P〈0.01）. Sperm motility rates of CP group were significantly lower than the normal group （P〈0.01）. After treated with ZBQQ decoction combined with Levofloxacin, sperm liquefaction time was signiticanfly shorter （P 〈 0.01）. Sperm motility rates were markedly elevated （P 〈 0.01）. Sperm apoptosis rates were markedly elevated in the chronic prostatitis patients compared with the healthy person （P〈0.01）. The levels declined significantly after treatment of ZBQQ decoction combined with Levofloxaein （P〈0.01）, while in the group treated with Levofloxacin, the distinction was not significant （P〉0.05）. The clinic cure rate of observe group was 35.71%, and overall effective rate was 85.71%, significantly higher than that of control group （P〈0.05）. Conclusion ZBQQ decoction combined with Levofloxacin can obviously improve the sperm quality, its possible mechanism maybe the depression of the sperm apoptosis rate.
