目的探讨ELISA双抗夹心一步法检测中的钩状效应。方法HBsAg初检用一步法,复检用二步法,对初检阴性而复检阳性血液标本先做中和确证试验,阳性者用一步法同厂家的二步法试剂检测,对检出阳性血液标本进行倍比稀释后用一步法和一步法同厂家的二步法试剂检测,直至一步法检出阳性。梅毒初检用一步法,复检用TRUST法,对初检阴性而复检阳性血液标本用TPPA法做确证试验,阳性者进行倍比稀释后用初检一步法检测,直至一步法检出阳性。结果初复检5 968份血液标本,共检出HBsAg阳性3份,于稀释度1∶2、1∶2、1∶4时分别检出;梅毒阳性2份,在稀释度1∶2、1∶4时分别检出。结论ELISA双抗夹心一步法检测献血者高浓度血液标本确有钩状效应,双抗夹心二步法无钩状效应,提高了血液安全质量。

Objective To probe into ELISA pair and resist hook form effect when a sandwich footwork was used. Methods On HBsAg examine,one footwork was used for the first time. Then two footwork was used. The positive blood sample was neutralized and proved conclusively. The positive one was measured with a footwork and two footwork reagents. And the positive samples were diluted and measured with a footwork and two footwork reagents until a footwork examined the masculine gender out. On syphilis examine, we used one footwork for the first time, examined again. We spent TRUST law to examine and proved with TPPA law conclusively. And the positive samples .were diluted and tested until a footwork examined the masculine gender out. Results 5 968 blood samples were examined,and 3 were HBsAg positive in dilution of 1 ： 2, 1 ： 2,1 ： 4. 2 were syphilis positive in dilution of 1 ： 2,1 ： 4. Conclusion The sandwich a footwork ELISA method really has a form effect of a hook on measuring the blood sample of blood donor with high density. The two sandwich footwork does not have form effect of hook and improve the quality of blood.
