超声诊断肝外胆管癌的价值

Diagnostic value of cholangiocarcinoma by ultrasonography

目的分析肝外胆管癌的声像图特点,探讨早期超声诊断的临床价值。方法50例超声拟诊为肝外胆管癌患者,经临床手术、其他影像学及(或)经皮胰胆管逆行造影证实,分析其患者临床及超声影像学资料。结果肝外胆管癌超声直接征象分为乳头型、团块型、截断型和狭窄型;间接征象为肝内及肝外胆管扩张、肝内及肝门部淋巴结转移和腹水。误诊3例,其中1例为胆总管结石,2例为十二指肠憩室。结论超声显像对肝外胆管癌的诊断有重要价值。

Objective To analyze the features of sonograms of eholangiocarcinoma, and to explore the clinical value of early ultrasonic diagnosis. Methods Fifty patients with suspected cholangiocarcnoma were diagnosed by ultrasonography, clinical and ultrasonic data of 47 cases confirmed by clinical operation, other imaging and ERCP were retrospectively analyzed. Results Ultrasonic direct signs of cholangiocarcinoma were divided into mammilla type, clumping type, tnmcation type and stenosis type; Indirect signs showed dilated intrahepatic and extrahepatic bile duct, liver metastasis, hepatic hilum lymph node invasion and ascites. Three cases were misdiagnosed, 1 case was calculus of common bile duct, 2 cases were duodenal diverticulum. Conclusion Ultrasonic imaging has important value for diagnosis in cholangiocarcinoma.
