第三代骨水泥技术在人工全髋置换术中的应用

Application of the Third Generation Cementing Technique in Total Hip Anthroplasty

摘要 目的探讨第三代骨水泥技术在人工全髋置换术中的临床应用。方法自2000年6月~2006年1月采用第三代骨水泥技术进行全髋关节置换术临床应用37例44髋,并分析其手术时间、总出血量、总输血量、术前与术后Harris评分及术周、术后并发症等。结果所有患者手术顺利进行,手术时间(84.5±6.3 min),总出血量(558.4±46.5 ml),平均输血量(332.5±21.5 ml),术前Harris评分(37.2±12.5),术后Harris评分(90.5±11.5),术后与术前Harris评分比较t=16.541,P<0.05。术后并发症,脱位1例,精神症状1例。本组病例无肺栓塞、深静脉血栓形成、感染等并发症发生。术后随访6.4~27.2月,平均12.6月,X线检查示假体位置良好。结论第三代骨水泥技术在人工全髋置换术中应用效果令人较为满意。 Objective To evaluate retrospectively the application of the third generation cementing technique in total hip arthroplasty(THA), and to determine the safety and effect of he third generation cementing technique. Methods Thirty seven patients(44 hips) had been performed total hip arthroplasty(THA) with the- third - generation cementing technique between June 2000 and January 2006. These had been analyzed the operating duration, the total amount of blood loss, the amount of blood transfusion necessary, pre - operation functional scores of hips and post - operation functional scores, the peri - operative and pest - operative complications and so on. Results The operations were performed successfully with the - third - generation cementing technique. The operat- ing duration averaged 84.5 ± 6.3min, the total amount of blood loss (558.4 ± 46.5 ) ml, the amount of blood transfusion necessary(332.5 ± 21.5)ml, pre - operation functional scores of hips 37.2 ± 12.5 and post - operation functional scores 90.5 ± 11.5. The difference had statistical significance between post - operation functional scores of hips and pre- operation functional scores( P 〈 0.05). The peri- operative and post- operative complications were happened to 2 cases, the dislocation of hips in 1 eases, and mental symptoms in 1 cases respectively, and no others complications in the series. The mean followed up period was 12.6 months, ranging from 6. 4 to 27.2 months, and the position of the components were radiographic satisfactory. Conclusion The third generation cementing technique cementing technique is rather effective in total hip anthroplasty.
