核因子-κB及细胞间黏附分子-1在OSAHS患者外周血单个核细胞的表达及意义——附30例报告 被引量:2

摘要 目的:探讨阻塞型睡眠呼吸暂停低通气综合征(obstructive sleep apnea-hypopnea syn-drome,OSAHS)患者外周血单个核细胞中核因子-κB(nuclear factor-κB,NF-κB)、细胞间黏附分子-1(intercellular adhesion molecule-1,ICAM-1)的表达水平及意义。方法:将30例确诊为OSAHS患者设为观察组,30例性别、年龄、体重指数与之相匹配的门诊非OSAHS患者设为对照组。2组均做多导睡眠图(polysomnography,PSG)检查,并采用流式细胞仪检测2组外周血中单个核细胞中NF-κB亚基p65及其下游产物ICAM-1的表达率;采用直线相关分析分析观察组治疗前NF-κB亚基p65及ICAM-1与PSG检测指标的关系。观察组接受持续正压通气治疗1个月,比较治疗前后NF-κB亚基p65及ICAM-1的表达水平变化。结果:与对照组比较,观察组的呼吸暂停低通气指数(apnea-hypopnea index,AHI)、SaO2低于0.90的时间占总睡眠时间百分比、SaO2下降幅度超过4%的次数均较高,NF-κB亚基p65及ICAM-1的表达率也较高(均为P(0.01)。相关分析结果显示,观察组NF-κB亚基p65、ICAM-1表达水平与上述3项PSG指标呈正相关(均为P(0.01);观察组20例患者完成1个月的持续正压通气治疗后NF-κB亚基p65以及ICAM-1水平均明显下降(均为P(0.01)。结论:OSAHS患者外周血单个核细胞NF-κB亚基p65及ICAM-1的表达显著增高,说明其可能参与OSAHS的病理生理过程。
