三尖瓣反流影响三尖瓣环运动的超声心动图研究

摘要 目的观察不同程度三尖瓣反流(TR)对三尖瓣环收缩期峰值流速(RV-Sm)、运动幅度(TAMA)与右室射血分数(RVEF)相关关系的影响。方法110例TR患者按反流程度分轻、中、重三组,接受超声心动图检查。其中30例(每组10例)进行了MRI右室造影。结果超声Simpson法RVEF与MRI法RVEF呈显著正相关。轻、中度TR组,RV-Sm与RVEF相关性良好(r分别为0.76,0.75),重度TR组RV-Sm与RVEF无相关性(r=0.2)。随着TR加重,TAMA出现了与RV-Sm一致的变化。结论重度TR影响三尖瓣环运动。重度TR时RV-Sm和TAMA不能反映RVEF。 Objective To investigate whether tricuspid regurgitation (TR) affects the relationship among peak systolic tricuspid annular velocity (RV- Sm), tricuspid annular motive amplitude (TAMA) and right ventricular ejection fraction (RVEF). Methods Totally 110 patients with TR underwent echocardiography and Doppler tissue studies. They were clas- sified as three groups: 50 patients with mild TR, 30 with moderate TR, and 30 with severe TR. Thirty patients were chosen to undergo magnetic resonance imaging right ventricular contrast. Results The RVEF estimated by Simpson's method corre- lated positively with that estimated by the MRI right ventricular contrast. RV - Sm showed a good correlation with RVEF in groups of mild or moderate TR (r- 0.76, r - 0.75). In severe TR group, RV - Sm had no correlation with RVEF (r - 0.2). Over a range of TR severities, the relationship between TAMA and RVEF showed a similar trend. Conclusions Severe TR has a significant impact on tricuspid annular movement. With severe TR , TAMA and RV- Sm can not represent right ventricular function.
