摘要
目的调查河北省农村不良妊娠结局分布状态。方法随机抽取全省11市50县的191个乡镇为调查单位,于2002年7月1日至2003年12月31日24,052例妇女自妊娠早期至婴儿出生满一周岁跟踪随访。结果不良妊娠结局总发生率为6.4%,其中廊坊1.7%、承德3.6%、秦皇岛4.6%、保定5.0%、石家庄5.8%、张家口6.7%、沧州7.3%、衡水8.0%、唐山8.2%、邢台8.5%、邯郸9.3%。畸形儿总发生率为94.0/万,其中廊坊5.7/万、承德51.0/万、秦皇岛95.8/万、保定88.0/万、石家庄74.4/万、张家口103.2/万、沧州40.0/万、衡水99.5/万、唐山73.1/万、邢台124.4/万、邯郸241.7/万。不良妊娠结局和畸形儿发生率廊坊最低、邯郸最高;不良妊娠结局和畸形儿发生率在不同地域明显不同,呈北到南逐渐增高趋势,南部发生率最高。结论河北省农村不良妊娠结局和畸形儿发生率有明显的地区差别,以南部发生率最高。
To understand the regional prevalence of abnormal pregnancy outcomes in rural pregnant women in Hebei province. Methods: A prospective epidemiological study with cluster random sampling was employed. A total of 24,052 early pregnant women in 191 towns which cover 11 cities all over Hebei province were randomly sampled from July 2002 to December 2003 and followed up until their infants were one year old. Results: The total incidence rate of abnormal pregnancy outcomes was 6.4% in Hebei. The regional incidence rate was 1.7%, 3.6%, 4.6%, 5.0%, 5.8%,6.7%, 7.3%, 8.0%,8.2%, 8.5%, 9.3% in the Langfang, Chengde, Qinhuangdao, Baoding, Shijiazhuang, Zhangjiakou, Cangzhou, Hengshui, Tangshan, Xingtai, Handan, respectively. The total incidence of congenital abnormality in Hebei province was 94.0 per ten thousand, while the regional rate was 5.7, 51.0, 95.8, 88.0, 74.4, 103.2, 40.0, 99.5, 73.1, 124.4, 241.7 per ten thousand in the Langfang, Chengde, Qinhuangdao, Baoding, Shijiazhuang, Zhangjiakou, Cangzhou, Hengshui, Tangshan, Xingtai, Handan, respectively. The incidence rates of abnormal pregnancy outcomes and congenital abnormality were the lowest in Langfang and the highest in Handan. There was a distinct regional disparity, with the prevalence gradually increasing from northern to southern Hebei. Conclusion: There was an obvious regional disparity in the prevalence of abnormal pregnancy outcomes in Hehei rural regions, with the highest incidence rate in southern Hebei.
