高分辨率CT观察腰椎椎体结构的骨质疏松性改变 被引量:8

摘要 目的:观察腰椎椎体结构的骨质疏松性改变。方法:采用高分辨CT横断扫描、定量CT,横断面CT值变化曲线、骨密度测定及三维重建等一系列CT技术对3名男性青年正常人以及23例临床骨质疏松患者的腰椎椎体内部结构进行对比分析。结果:骨质疏松患者骨皮质的厚度以及骨密度减小,质地不均匀与退行性变并存,并具有塑形改变;骨松质改变具有特征性,表现为均匀性混乱,垂直骨小梁数量减少,间距增加,骨小梁倾斜变形,横行骨小梁数量明显减少,出现很多的残端。结论:高分辨率CT为骨质疏松症的早期诊断、骨折危险性预测和疗效监控等提供了具有临床应用价值的手段。 Objective: To observe structure changes in osteoporostic lumbar vertebraI body. Methods: By means of a series of CT techniques, such as HRCT, QCT, CT profile, SEQCT BMD analysis, 3 young volunteers and 23 patients with osteoporosis were observed and compared. Results: CT scan indicated that thickness and BMD of cortical bone of vertebral body decreased,accompanied by degenerative and plastic changes; trabecular bones were characterized by disarrangement of trabecular network, decrease of trabecular length and number of trabeculae, increase of width of intertrabecular space, tilted and disformed and trabeculae, and marked decrease of transverse trabeculae with free ends. Conclusion: HRQCT is a clinically practical method of early detection, diagnosis and treatment of osteoporosis.
