外伤性眼球脱臼临床分析

The clinical features of traumatic luxation of the eyeball

目的探讨外伤性眼球脱臼的发病机制、临床表现和处理原则。方法外伤性眼球脱臼6例(6眼),其中眼球半脱臼5例,眼球全脱臼1例,进行临床分析。结果眼球半脱臼5例中,4例合并眶壁骨折,2例合并巩膜裂伤。5例均行巩膜伤口探查及眼球还纳术。术后除1例保留颞侧光感外,其余均无光感。眼球全脱臼者合并眶壁骨折,无法还纳,行眼球摘除术。结论外伤性眼球脱臼伤情复杂,可合并眶壁骨折和巩膜裂伤,预后差。

Objective To investigate the mechanism, clinical features and treatment principle of traumatic luxation of the eyeball. Methods 6 cases of traumatic luxation of the eyeball in 2006 were analyzed retrospectively. In which 5 cases were semi-luxation and 1 case was total luxation. Results 4 cases were complicated with orbital fracture and 2 cases with scleral laceration in the 5 cases of semi-luxation of the eyeball. All the 5 cases received scleral exploration and eyeball reposition surgery. They had no light perception except for 1 case remaining light perception of temporal side after operation, The one case of total luxation of the eyeball was complicated with orbital fracture and could not be reposited. He received eyeball enucleation. Conclusion Traumatic luxation of the eyeball is complex and may be complicated with orbital fracture and scleral laceration therefore with poor prognosis.
