国产吻合器痔上黏膜环切术治疗重度痔238例疗效观察

Home-made stapler for PPH for hemorrhoids in 238 cases

目的评价国产吻合器在治疗Ⅲ-Ⅳ期脱出性内痔的临床效果。方法利用国产痔上吻合器对238例重度脱出痔患者进行手术治疗。结果238例重度痔患者应用国产吻合器，吻合均一次成功，术后患者痔核完全消失。有32例术后并发少量出血（13．4％），经保守治疗痊愈。术后并发尿潴留152例（63．8％），肛门轻度疼痛56例（23．5％），下腹部不适28例（11．7％）。随访12～24个月，无肛门狭窄，无大便失禁。结论应用国产吻合器行痔上黏膜环切术治疗重度痔患者术后痛苦小、恢复快、复发率低、并发症少、副损伤小。

Objective To observe the clinical treatment of prolapse Ⅲ- Ⅳ phase internal Hemorrhoid with home-made Stapler. Methods Home-made Stapler was used for 238 severe prolapsed hemorrhoids cases, including 136 male patients and 102 female patients. The age range was from 23 years old to 72 years old, with an average 47.3 years. The treatment period was 2-30 years. Results The surgery time was 7-10 minutes, and the postoperative hospitalization was 3 days. 8% patient were satisfied with surgery. 32 patients （ 13.4% ） had mild bleeding after surgery and recovered after conservative therapy. 152 patients （63.8%） had uroschesis and 56 patients （23.5%） had a little ache of anus, meanwhile 28 patients （11.7%） has some abdomen discomfort. We visited these patient at random in following 12-24 months and no patients had anus straitness and dejecta incontinence. Conclusion Home-made stapler for PPH has some advantages which include little postoperative pain, quick recovery , low recurrence, less trauma and complications.
