椎管内肿瘤的显微外科治疗(附40例报告)

Microsurgical Treatment of Intraspinal Tumors (a Report of 40 Cases)

目的探讨椎管内肿瘤显微外科手术治疗方法和效果。方法对40例椎管内肿瘤（位于颈段8例，胸段16例，腰骶段10例，马尾区6例）患者的临床资料进行回顾性分析。结果38例肿瘤全部切除，2例肿瘤大部切除。随访2—5年，疼痛缓解率为88．6％（31／35），感觉障碍改善率76．3％（29／38），运动障碍改善率90．5％（19／21），括约肌功能改善率88．9％（16／18）。结论椎管内肿瘤以良性为多，重视显微外科技术的应用和脊柱稳定性的重建是提高其疗效的关键。

Objectives To investigate the questions related to microsurgery for intraspinal tumors and to evaluate the operative outcomes. Method The clinical data of 40 patients with intraspinal tumors were analysed retrospectively. Results Of 40 cases of intraspinal tumors, 8 were in cervical cord, 16 in thoracic cord and 10 in lumbar cord and 6 in sacral cord. Of 40 cases of intraspinal tumors, 38 were totally resected and 2 subtotally. The following up from 2 to 5 .years showed that pain was relieved in 88.6% （31/35） of the patients, sense function was improved in 76.3% （29/38）, motor function was improved in 90.5% （19/21）, and sphincter function was improved in 88.9% （16/18）. Conclusions The curative effect of microsurgery on the intraspinal tumors, of which, most were benign, is good. The skillful microsurgical technique and reconstruction of spinal stability are key to good curative effect in the patients with intraspinal tumors.
