摘要
目的探讨血压调控达标对原发性高血压患者动脉硬化度的影响。方法应用脉搏波传导速度自动测量系统测定颈动脉-股动脉脉搏波速度(carotid-femoral pulse wave velocity,C-FPWV),作为反映动脉硬化度的参数。对69例高血压病降压达标患者、56例高血压病降压未达标患者和54例健康体检者进行了C-FPWV检测。结果降压未达标的高血压患者血管硬化度[(12.70±2.82)m/s]明显高于健康人的血管硬化度[(11.23±2.89)m/s](P<0.05),也明显高于降压达标者[(11.96±3.62)m/s](P<0.05)。降压达标的高血压患者的动脉硬化度高于健康人的动脉硬化度(P<0.05)。多元逐步回归分析结果表明年龄、脉压与反映大动脉硬化度的C-FPWV成正相关(P<0.01),回归方程为PWV(m/s)=4.68+0.09×年龄(岁)+0.04×脉压(mm Hg)。结论通过对血压的调控,将血压控制达标可以改善高血压患者的动脉硬化度。年龄、脉压与大动脉硬化度密切相关。
Objective To explore the change of arterial stiffness in primary hypertension patients with reasonable treatment. Methods Carotid-femoral pulse wave velocity (C-FPWV) measured by pulse wave conduction speed automatic measurement system reflects the artery stiffness as the parameter. Pulse wave velocity were measured in sixty-nine primary hypertension patients with reasonable treatment, fifty-six hypertension patients with unreasonable treatment and fifty-four people with health examination. Results The arterial stiffness of hypertension patients with unreasonable treatment ( 12.70 ± 2.82 ) m/s was obviously higher than that of the people with health examination ( 11.23 ± 2.89 ) m/s ( P 〈 0.05 ), and was obviously higher than that of the hypertension patients with reasonable treatment ( 11.96 ± 3.62 ) m/s ( P 〈 0.05 ). The arterial stiffness of hypertension patients with reasonable treatment was obviously higher than that of the people with health examination ( P 〈 0.05 ). Multiple stepwise regression analysis show that C-FPWV was positively correlated with age and pulse pressure( P 〈0.01 ). Its multiple linear regression equation was C-FPWV = 4.68 ± 0.09 ×( age ) ± 0.04 × (PP). Conclusions The arterial stiffness can decrease by reasonable treatment. PWV was closely related to age and pulse pressure.
