摘要 目的探讨胸腔积液G-抗原(GAGE)基因表达情况及其在鉴别诊断良、恶性中的意义。方法收集胸腔积液97例,分为A、B、C和D四组,其中A组为结核性胸腔积液组,共41例;B组为其它病因组共10例;C组为恶性胸腔液细胞学检查阳性组,共40例;D组为恶性胸腔液细胞学检查阴性组,共6例。A组与B组合并为良性胸腔积液组,C组与D组合并为恶性胸腔积液组,采用通用引物巢式逆转录聚合酶链反应(RT-PCR)方法检测各组标本GAGE基因表达。结果GAGE基因在良、恶性胸腔积液阳性率分别为3.9%(2/51)、60.9%(28/46),表达差异有统计学意义(P<0.05)。结论GAGE基因在鉴别良、恶性胸腔积液中体现出较高的敏感性和特异性,有望成为鉴别良、恶性胸腔积液有效的检测方法之一。 Objective To investigate the expression of GAGE gene in the pleural effusion and evaluate the diagnostic significance of GAGE gene in benign and malignant pleural effusion. Methods 97 cases of pleural effusion were collected and divided into four groups:group A (tubercular pleural effusion ,41 cases) ;group B (10 cases, including 4 cases of Cardiogenic pleural effusion,5 cases of inflammatory pleural effusion, 1 case of hepatogenic pleural effusion) ;group C( pleural effusion of malignancy with positive cytological examination,40 cases) ;group D( pleural effusion of malignancy with negative cytological examination,6 cases). Group A and group B were viewed as group of benign pleural effusion;and group C and group D constituted to the group of malignant pleural effusion. The expression of GAGE gene in each specimen was amplified by RT-nested PCR with GAGE common primers. Results The results showed that the positive rate of GAGE gene expression in the benign and malignant pleural effusion were 3.9% (2/51) and 60.9% (28/46) respectively,with statistical significanee(P 〈0.05). Conclusions It is suggested the expression of GAGE gene has the high sensitivity and specificity in identifying the benign and malignant pleural effusion. It possibly becomes an effectively diagnostic tool for identifying the benign and malignant pleural effusion.
