摘要
目的比较Nuss手术、改良ravitch手术及胸骨翻转手术对漏斗胸的矫正效果。方法1995年5月至2008年7月手术并随诊的32例漏斗胸病例,其中Nuss手术15例,改良ravitch手术9例,胸骨翻转手术8例,将三种手术方式的一般情况、围手术期情况、并发症、效果等进行比较,同时对行Nuss手术的15例病例中的对称组(9例)和不对称组(6例)术后效果进行比较。结果32例均顺利完成手术,三组患者中平均年龄无差异,但平均手术时间、术中出血量、引流时间、引流量、术后住院天数Nuss组明显少于改良ravitch组及胸骨翻转组(P<0.05),Nuss组术后效果优良率优于其它两组,疼痛时间Nuss组较改良ravitch组及胸骨翻转组长,Nuss手术对对称型漏斗胸矫正效果优于不对称型漏斗胸。结论Nuss手术与其他两种手术方式相比具有切口隐蔽、操作简便、手术时间短、术中出血少、创伤小等优点,Nuss手术尤其适用于广泛对称型漏斗胸。
Objective To compare the efficacies of nuss procedure, modified ravitch procedure and sternoturnover procedure. Methods 32 cases of patients with peetus excavatum repairs were followed up .Among them,there were 15 nuss procedures, 9 modified ravitch procedures and 8 sterno-turnover procedures.The conditions before, during and after operation, complications and effects were compared to evaluate the efficacies of nuss procedure on repairing symmetric or asymmetric pectus excavatum. Results Among the three groups,there was no difference in age, but the operative time, blood loss, duration of drainage, amount of drainage were less in nuss group （P〈0.05）. Duration of pain in nuss group was not longer than the other groups. The effect of nuss group was the best one in the three groups. Conclusion Nuss procedure is easy to be performed with minimal invasion, the duration of drainage, amount of drainage, operative time, blood loss are less than the other groups. Nuss procedure is suitable for the symmetric pectus excavatum.
