上颌前牙种植体周围牙龈美学重建的临床观察(附15例报告)

Clinical observation of new soft tissues around implant-support prosthetics in the anterior maxilla:Report of 15 cases

目的评价上颌前牙种植体周围牙龈美学重建的效果。方法选择30例上颌前牙缺失患者随机分两组,实验组15例在牙槽嵴顶植入种植体,并在周围骨缺损区植骨,建立骨支架;转移瓣与滑行瓣重建种植体周围牙龈,精确比色,精度烤瓷修复。对照组15例常规植入种植体,植骨,基台牙龈成形,比色,烤瓷修复。两组均分别于种植修复后3、6、12个月应用划卡满意度评价技术,观察种植体周围牙龈美学重建的效果。结果实验组种植修复后牙龈美学评价情况与对照组比较均有统计学意义(P<0.05)。12个月实验组满意度80%,对照组40%;实验组不满意度7%,对照组40%。结论上颌前牙种植体周围牙龈美学重建效果显著。

Objective To evaluate the effect of new gingiva around implant-support prosthetics in the anterior maxilla. Methods Thirty adult patients without teeth in the anterior maxilla were divided into two groups, including experimented group and controlled group. Fifteen implants were placed into ridge bone and Bio-Oss was applied to the bone defects around the implants. The soft tissues around the implants were managed in experimented group. The patients of controlled group were gone standard dental implant placements, second stage surgery. The effect were observed at three, six, and 12 months after dental implant prosthetics. Results The well-esthetic result of experimented group was 80%, and that of controlled group was 40% ; No esthetic result of experimented group was 7%, and that of controlled group was 40%. The esthetic result of experimented group was better than the controlled （P〈0.05）. Conclusion The clinical result of new gingiva around implant-support prostheties in the anterior maxilla is good.
